【摘要】：目的:探討產(chǎn)前超聲及MRI在診斷胎兒顱后窩異常中的應用價值及臨床意義。方法:對2011年1月—2012年12月于我院行超聲檢查擬診胎兒顱后窩異常者進行回顧性分析,隨訪其產(chǎn)前超聲表現(xiàn)、MRI檢查結(jié)果、妊娠結(jié)局及產(chǎn)后影像學資料。結(jié)果:研究期間超聲檢出胎兒顱后窩異常并同時進行MRI檢查共27例,其中18例(66.7%)MRI證實了超聲診斷,另9例MRI顯示胎兒顱后窩結(jié)構(gòu)正常,產(chǎn)后隨訪亦未見異常。其中11例活產(chǎn),10例進行了產(chǎn)后MRI檢查,其中5例(50%)產(chǎn)前、產(chǎn)后MRI結(jié)果完全相符,2例(20%)兩者診斷不相符,另3例(30%)產(chǎn)后MRI補充了新的診斷。結(jié)論:產(chǎn)前超聲對發(fā)現(xiàn)胎兒顱后窩異常有重要的臨床意義,MRI對病變的顯示效果及診斷符合率較超聲更精確,可作為重要的驗證和補充診斷手段,但應重視產(chǎn)后的影像學隨訪觀察。
[Abstract]:Objective: To investigate the prenatal ultrasound and MRI fossa and its clinical significance in the diagnosis of abnormal value of fetal posterior fossa. Methods: January 2011 - December 2012 anomaly in our hospital ultrasound examination for suspected fetal posterior who were retrospectively analyzed and followed the prenatal ultrasound, MRI examination results, the outcome of pregnancy and postpartum imaging results: during the study period. Ultrasound detection of fetal posterior fossa abnormalities and MRI was performed in 27 cases, including 18 cases (66.7%) MRI confirmed the ultrasound diagnosis, the other 9 cases MRI showed fetal posterior fossa structures, postpartum follow-up showed normal. 11 cases of live births, 10 cases of postpartum MRI check, in which 5 cases (50%) prenatal, postpartum MRI were consistent in 2 cases (20%), the diagnosis does not match, the other 3 cases (30%) postpartum MRI adds new diagnosis. Conclusion: prenatal ultrasound has important clinical significance for the detection of fetal anomalies of posterior fossa lesions on MRI. The display effect and diagnostic coincidence rate are more accurate than ultrasound, which can be used as an important means of verification and supplementary diagnosis. However, attention should be paid to the follow-up and observation of postpartum imaging.
