【摘要】：目的探討經(jīng)皮經(jīng)肝超聲造影顯像在評估兔VX2肝癌腹腔淋巴轉(zhuǎn)移中的應(yīng)用價值。方法新西蘭大白兔30只,分為正常對照組(A組)5只,肝癌種植組(B組)25只,同時在癌組織及癌周組織注射超聲造影劑Sono Vue,動態(tài)觀察正常肝組織、癌組織及癌周組織的增強(qiáng)特點,并使用超聲儀器自帶造影分析軟件定量分析。開腹清掃淋巴結(jié)并送病理學(xué)檢查。結(jié)果 25只肝癌種植組(B組)中,無淋巴結(jié)轉(zhuǎn)移(B1組)14只,淋巴結(jié)轉(zhuǎn)移(B2組)11只。肝臟經(jīng)皮經(jīng)肝超聲造影顯像分為組織間隙期、彌散期、平臺期、消退期4期,癌組織與癌周組織比較,超聲造影劑在癌組織呈現(xiàn)為"快退",癌周組織能夠持續(xù)增強(qiáng)20 min以上并逐漸消退。平臺期強(qiáng)度(PTI)表現(xiàn)為癌組織、癌周組織及正常肝組織間差異有統(tǒng)計學(xué)意義(P0.01),且PTI癌組織PTI癌周組織PTI正常肝組織。B1、B2組癌周組織及A組正常肝組織各組間的PTI差異均有統(tǒng)計學(xué)意義(P0.01),且PTIB2PTIB1PTIA。B1組、B2組癌組織的PTI與A組比較差異有統(tǒng)計學(xué)意義(P0.05),但B1、B2兩組間差異無統(tǒng)計學(xué)意義。結(jié)論經(jīng)皮經(jīng)肝超聲造影顯像可提供肝癌腹腔淋巴轉(zhuǎn)移情況的信息,為選擇治療方式及分析預(yù)后提供重要依據(jù)。
[Abstract]:Objective to evaluate the value of percutaneous transhepatic ultrasound imaging in evaluating the abdominal lymphatic metastasis of VX2 hepatoma in rabbits. Methods Thirty New Zealand white rabbits were divided into normal control group (n = 5) and hepatoma implantation group (n = 25). At the same time, ultrasound contrast agent Sono Vue was injected into cancer tissue and pericancerous tissue to observe the enhancement characteristics of normal liver tissue, cancer tissue and pericancerous tissue dynamically. The lymph nodes were dissected by laparotomy and sent to pathological examination. Results in group B of 25 patients with liver cancer, 14 were in group B with no lymph node metastasis, 14 in group B without lymph node metastasis, and 14 in group B without lymph node metastasis. There were 11 cases in group B _ 2 of lymph node metastasis. The liver was divided into 4 stages by percutaneous transhepatic contrast-enhanced ultrasound imaging: interphase, diffuse stage, plateau stage, and extinction phase. The cancer tissue was compared with the pericancerous tissue. Ultrasound contrast agent showed "rapid regression" in cancer tissue, which could be continuously enhanced for more than 20 min and gradually receded. There was significant difference between the pericancerous tissues and normal liver tissues (P 0.01), and there was significant difference in PTI between the surrounding tissues of PTI and the normal liver tissues of group A and the surrounding tissues of PTI in PTI tissues of PTI cancer tissues, and the PTI of normal liver tissues in group A was significantly different from that in group PTIB2PTIB1PTIA.B1, and there was a significant difference in PTI in group B 2 and group B 2 in PTIB2PTIB1PTIA.B1 group. The difference of PTI between group A and group A was statistically significant (P 0.05), but there was no significant difference between group B 1 and B 2. Conclusion Percutaneous transhepatic contrast-enhanced imaging can provide information of lymphatic metastasis in abdominal cavity of hepatocellular carcinoma. To provide an important basis for the selection of treatment methods and analysis of prognosis.
