【摘要】：目的評估拉伸指數(shù)模型擴散加權(quán)成像(DWI)參數(shù)在鑒別腎臟乏脂性錯構(gòu)瘤與乳頭狀細(xì)胞癌臨床價值。方法回顧性分析47例經(jīng)手術(shù)病理證實腎乏脂性錯構(gòu)瘤(n=21)與乳頭狀腎細(xì)胞癌(n=26)患者臨床資料,所有患者在3.0 T MRI采用多b值DWI(b=0~1700 s/mm~2)進行掃描�；诶熘笖�(shù)模型計算相應(yīng)的定量參數(shù)[擴散分布系數(shù)(DDC),擴散異質(zhì)性(α)]值。在兩者之間鑒別采用獨立樣本t檢驗進行分析,采用組內(nèi)相關(guān)系數(shù)(ICC)及接受者工作特征曲線(ROC)進行統(tǒng)計評估。結(jié)果與腎乏脂性錯構(gòu)瘤[DDC=(3.061±2.729)×10~(-3)mm~2/s]相比,腎乳頭狀細(xì)胞癌中DDC明顯降低[DDC=(0.953±0.417)×10~(-3)mm~2/s,P0.001],而α值(α=0.776±0.113)明顯高于乏脂性錯構(gòu)瘤(α=0.412±0.088,P0.001)。拉伸指數(shù)模型DWI定量參數(shù)值在兩者之間鑒別診斷的曲線下面積(AUC)、敏感性、特異性和最佳截止值分別為:α:0.998、96.3%、100.0%、0.595;DDC:0.882、91.3%、74.1%、1.202×10~(-3)mm~2/s。結(jié)論腎乏脂性錯構(gòu)瘤與乳頭狀腎細(xì)胞癌具有不同的拉伸指數(shù)擴散定量參數(shù)值;α值在鑒別兩者具有較高的診斷效能。
[Abstract]:Objective to evaluate the clinical value of DWI parameters in differentiating renal hypolipidemic hamartoma from papillary cell carcinoma. Methods 47 cases of renal hypolipidemic hamartoma proved by surgery and pathology were retrospectively analyzed. Clinical data of patients with renal cell carcinoma. All patients were scanned at 3.0T MRI with multiple b DWI(b=0~1700 s / mmm2). The corresponding quantitative parameters were calculated based on the stretching index model [diffusion distribution coefficient (DDC), diffusion heterogeneity (偽)]. Independent sample t test was used to analyze the difference between the two. The results were compared with that of renal hypolipidemic hamartoma (DDC=(3.061 鹵2.729) 脳 10 ~ (-3) mm ~ (-3) / s, compared with that of renal hypolipidemic hamartoma (DDC=(3.061 鹵2.729) 脳 10 ~ (-3) mm ~ (-3) / s. DDC was significantly decreased in renal papillary cell carcinoma [DDC=(0.953 鹵0.417) 脳 10 ~ (-3) mm ~ (-2) 路s ~ (-1) P ~ (0.001)], and 偽 value (偽 ~ (0.776 鹵0.113)) was significantly higher than that of hypolipidemic hamartoma (偽 _ (0.412 鹵0.088) P _ (0.001)). The specificity and the best cut-off value were: 偽: 0.998 / 96.3and 100.00.595DDCW 0.882/ 91.3 / 74.1and 1.202 脳 10 ~ (-3) mm ~ (-2) / s respectively. Conclusion there are different quantitative parameters of extension index diffusion between renal hypolipidemic hamartoma and papillary renal cell carcinoma, and a value has higher diagnostic efficacy in differentiating them.
