【摘要】：目的分析急性大面積腦梗死患者血栓的磁敏感加權(quán)成像(SWI)特點(diǎn)及其與臨床的相關(guān)性。方法選擇46例急性大面積腦梗死(臨床癥狀出現(xiàn)24 h)患者進(jìn)行SWI檢查,并在2周、3個(gè)月進(jìn)行隨診。分析急性大面積腦梗死血栓的SWI圖像特點(diǎn),并得出相應(yīng)美國(guó)國(guó)家健康研究所腦卒中分級(jí)量表(NIHSS)評(píng)分,進(jìn)行相關(guān)統(tǒng)計(jì)學(xué)分析。結(jié)果有34例顯示血栓存在,占74%。其中急性期和亞急性期(2周)顯示32例,穩(wěn)定恢復(fù)期(3個(gè)月)顯示2例,不同時(shí)期內(nèi)血栓的顯示特點(diǎn)差異有統(tǒng)計(jì)學(xué)意義(P0.05)。其中血栓長(zhǎng)度≥4 cm 23例,血栓長(zhǎng)度與臨床NIHSS評(píng)分有相關(guān)性(r=0.783,P0.01)。血栓位于梗死血管起始部26例,遠(yuǎn)側(cè)部8例,血栓的位置與臨床NIHSS評(píng)分有相關(guān)性(r=0.747,P0.01)。結(jié)論 SWI對(duì)無創(chuàng)顯示急性大面積腦梗死血管內(nèi)血栓具有敏感性和特異性;量化分析有助于判斷急性大面積腦梗死的演變情況,對(duì)臨床治療及預(yù)后評(píng)估起到一定的指導(dǎo)作用。
[Abstract]:Objective to analyze the characteristics of magnetic sensitivity weighted imaging (SWI) and its correlation with clinical manifestations in patients with acute large area cerebral infarction. Methods 46 patients with acute large area cerebral infarction (24 hours after clinical symptoms) were examined by SWI. The characteristics of SWI images of acute large area cerebral infarction thrombosis were analyzed and the corresponding NIHSS scores were obtained by the stroke grading scale of the National Institute of Health of the United States of America. Results there were 34 cases of thrombus, accounting for 74%. Among them, 32 cases were found in acute and subacute phase (2 weeks), and 2 cases in stable recovery period (3 months). There were significant differences in the characteristics of thrombus in different periods (P 0.05), among which 23 cases had thrombus length 鈮，
