【摘要】：目的探討超聲引導(dǎo)下胸膜活檢結(jié)合二維超聲(2DUS)在良惡性胸膜病變?cè)\斷中的應(yīng)用價(jià)值。方法回顧性分析193例胸膜病變患者的胸膜活檢病理結(jié)果、二維超聲及臨床資料,對(duì)比良、惡性胸膜病變的二維超聲表現(xiàn),分析取材成功率及穿刺活檢對(duì)胸膜良、惡性病變?cè)\斷的靈敏度,探討超聲引導(dǎo)下胸膜穿刺活檢結(jié)合二維超聲在胸膜良惡性病變?cè)\斷中的應(yīng)用價(jià)值。結(jié)果胸膜活檢成功率為79.3%(153/193),病理確診率為84.1%(149/182),其中惡性胸膜病變80.0%,結(jié)核性胸膜炎64.8%,胸膜非特異性炎癥63.6%。良性組與惡性組胸膜平均厚度分別為(0.61±0.22)cm和(0.74±0.39)cm,兩者有統(tǒng)計(jì)學(xué)意義(P=0.031);良性組與惡性組胸水平均深度分別為(2.95±1.89)cm和(4.42±2.87)cm,兩者有統(tǒng)計(jì)學(xué)意義(P=0.001)。結(jié)論良、惡性胸膜病變?cè)?DUS上存在一定的聲像學(xué)特征,穿刺活檢前詳細(xì)評(píng)估其特征可提高胸膜病變超聲診斷率和活檢陽性率。
[Abstract]:Objective to evaluate the value of ultrasound guided pleural biopsy combined with two-dimensional ultrasound 2DUSin the diagnosis of benign and malignant pleural diseases. Two-dimensional ultrasound findings of malignant pleural lesions. The success rate of sampling and the sensitivity of puncture biopsy in the diagnosis of benign and malignant pleural lesions were analyzed. To explore the value of ultrasound guided pleural biopsy combined with two-dimensional ultrasound in the diagnosis of benign and malignant pleural diseases. Results the successful rate of pleural biopsy was 79.3 / 153 / 193, and the rate of pathological diagnosis was 84.1 / 182 / 149 / 182, among which the malignant pleural lesions were 80.0% and tuberculous pleurisy was 64.8%. The mean pleural thickness of benign group and malignant group was 0.61 鹵0.22 cm and 0.74 鹵0.39 cm, respectively, which had statistical significance, and the depth of pleural level in benign group and malignant group were 2.95 鹵1.89 cm and 4.42 鹵2.87 cm, respectively. Malignant pleural lesions have some sonographic features on 2DUS. The detailed evaluation of the features before biopsy can improve the diagnostic rate of pleural lesions and the positive rate of biopsy.
