【摘要】：目的探討原發(fā)性性腺外內(nèi)胚竇瘤(EST)的影像學(xué)特征及其病理基礎(chǔ),提高對該腫瘤的認識。方法回顧性分析17例經(jīng)病理證實的原發(fā)性性腺外EST的臨床及影像學(xué)資料,總結(jié)其臨床、CT及MRI特征,并與手術(shù)病理結(jié)果進行對照分析。結(jié)果腫瘤位于前縱隔5例,腹膜后1例,骶尾部8例,顱腦3例。男7例,女10例。CT及MRI表現(xiàn)為巨大軟組織腫塊,呈圓形、橢圓形或不規(guī)則形,密度及信號不均勻,13例為實性腫塊,4例為囊實性腫塊;增強掃描實性部分呈中度-高度強化,囊性部分無強化,10例瘤體內(nèi)可見走行迂曲的線狀或條狀血管影。17例行血清學(xué)檢查血清甲胎蛋白(AFP)明顯升高。病理學(xué)表現(xiàn)為腫瘤組織形態(tài)多樣,間質(zhì)排列成疏松的網(wǎng)狀結(jié)構(gòu),其內(nèi)可見S-D小體,嗜酸性小體。結(jié)論原發(fā)性性腺外EST影像表現(xiàn)具有一定的特征性,再結(jié)合患者發(fā)病年齡、性別、部位及AFP檢查等資料有助于提高其術(shù)前診斷的準(zhǔn)確性,但確診仍需依賴病理。
[Abstract]:Objective to investigate the imaging features and pathological basis of primary extragonadal endodermal sinus tumor (est) and to improve the understanding of the tumor. Methods the clinical and imaging data of 17 cases of pathologically proved primary extragonadal EST were retrospectively analyzed. The clinical features of CT and MRI were summarized and compared with the results of operation and pathology. Results the tumors were located in anterior mediastinum in 5 cases, retroperitoneal in 1 case, sacrococcygeal in 8 cases, craniocerebral in 3 cases, male 7 cases, female 10 cases. Ct and MRI showed large soft tissue mass. Round, oval or irregular shape, density and signal intensity inhomogeneity in 13 cases, solid mass in 4 cases, solid mass in 4 cases, solid part of enhanced scan showed moderate to high enhancement, In 10 cases of cystic partial unenhanced tumor, the serological examination of serological examination showed that the AFP of serum alpha-fetoprotein was significantly increased in 10 cases. The pathological manifestation was that the tumor tissue was diverse, and the interstitial cells were arranged into loose reticular structure. S-D corpuscles and eosinophilic corpuscles were found in the patients. Conclusion the imaging features of primary extragonadal EST have some characteristics. Combining with the age, sex, site and AFP examination of the patients, it is helpful to improve the accuracy of preoperative diagnosis. But the diagnosis still depends on pathology.
