【摘要】：目的探討聲觸診組織量化(VTQ)技術(shù)無(wú)創(chuàng)評(píng)價(jià)肝纖維化程度的臨床應(yīng)用價(jià)值。方法 62例肝纖維化患者(S1~S3期組)、36例肝硬化患者(S4期組)及50例健康志愿者(對(duì)照組),應(yīng)用VTQ技術(shù)定量檢測(cè)肝實(shí)質(zhì)剪切波速度。結(jié)果對(duì)照組、S1~S3期組及S4期組患者剪切波速度分別為(1.11±0.19)m/s、(1.62±0.41)m/s及(2.48±0.53)m/s,三者比較差異有統(tǒng)計(jì)學(xué)意義(P㩳0.05)。S1~S3期組ROC曲線下面積為0.801,S4期組ROC曲線下面積為0.956。剪切波速度以1.30m/s為界值,診斷S1~S3期的敏感性為81%,特異性為79%;以1.81m/s為界值,診斷S4期的敏感性為92%,特異性為85%。結(jié)論 VTQ可無(wú)創(chuàng)測(cè)量肝實(shí)質(zhì)剪切波速度,客觀反映肝組織的彈性模量,有望為定量評(píng)估肝纖維化和肝硬化提供一種客觀、準(zhǔn)確的新方法。
[Abstract]:Objective to investigate the clinical application value of acoustic palpation tissue quantification (VTQs) technique in noninvasive assessment of hepatic fibrosis. Methods Sixty-two patients with hepatic fibrosis were treated with S1S3 phase group and 36 patients with cirrhosis with S4 stage) and 50 healthy volunteers (control group). Results the shear wave velocities were 1.11 鹵0.19 m / s and 2.48 鹵0.53 m / s in the control group and S4 group, respectively. There was a significant difference among the three groups (P < 0.05). The shear wave velocities of liver parenchyma were measured by VTQ technique, and the results showed that the shear wave velocities in the control group were 1.11 鹵0.19 m / s and 2.48 鹵0.53 m / s, respectively, and there were significant differences among the three groups (P < 0.05). The area under the ROC curve of stage 0.801and S4 was 0.956. The sensitivity of diagnosing S1 / S3 was 81m / s, the specificity was 79g / s, the value of shear wave velocity was 1.30m / s, and the value of 1.81m / s was taken as the boundary value, and the area under the ROC curve was 0.801m / s and 0.801m / s respectively, and the sensitivity of S1S3 was 81m / s, and the specificity was 79g / s. Conclusion VTQ can measure the shear wave velocity of hepatic parenchyma and objectively reflect the elastic modulus of liver tissue. It is expected to provide an objective and accurate new method for quantitative evaluation of liver fibrosis and cirrhosis.
