主動脈弓形態(tài)在診斷胎兒法洛四聯(lián)癥中的應(yīng)用

【摘要】:目的探討產(chǎn)前超聲觀察胎兒主動脈弓形態(tài)對法洛四聯(lián)癥(TOF)的診斷價值。方法收集2012年10月—2014年10月收治的TOF胎兒109胎(TOF組),另選正常胎兒97胎(對照組),回顧性分析產(chǎn)前超聲檢查主動脈弓切面動態(tài)圖像,計算最大曲率位置,對比兩組間差異,并探討最大曲率位置與孕齡的關(guān)系。結(jié)果對照組最大曲率位置為(65.63±11.01)%,TOF組為(43.28±9.10)%,差異有統(tǒng)計學(xué)意義(t=8.95,P0.01)。以47.18%為診斷胎兒法洛四聯(lián)癥的最優(yōu)最大曲率位置點,此時敏感度為97.13%,特異度為73.50%。對照組最大曲率位置與孕齡呈正相關(guān)(r=0.776,P0.05);TOF組最大曲率位置與孕齡呈負(fù)相關(guān)(t=-0.537,P0.05)。結(jié)論胎兒主動脈弓形態(tài)的超聲表現(xiàn)對豐富胎兒TOF的診斷信息和提高診斷率具有重要意義。
[Abstract]:Objective to investigate the diagnostic value of fetal aortic arch observation by prenatal ultrasound in tetralogy of Fallot. Methods from October 2012 to October 2014, 97 normal fetuses (control group) were collected. Antepartum ultrasound was used to analyze the dynamic images of aortic arch. Calculate the position of maximum curvature, compare the difference between the two groups, Results the maximum curvature position of the control group was 65.63 鹵11.01 and the maximum curvature position of TOF group was 43.28 鹵9.100.The difference was statistically significant. The best maximum curvature of tetralogy of Fallot was 47.18%. The sensitivity was 97.13 and the specificity was 73.50. The position of maximum curvature in the control group was positively correlated with gestational age. There was a negative correlation between the position of maximum curvature and gestational age in the control group. Conclusion the ultrasonic findings of fetal aortic arch are valuable in the diagnosis of fetal TOF. And improve the diagnostic rate is of great significance.
