【摘要】：目的:探討脊柱惡性血管源性腫瘤的影像表現(xiàn)。方法:對(duì)9例經(jīng)病理確診的脊柱惡性血管源性腫瘤的CT和MRI表現(xiàn)進(jìn)行回顧性分析并總結(jié)其影像特點(diǎn)。結(jié)果:9例中2例為血管肉瘤,7例為上皮樣血管內(nèi)皮瘤;5例病變位于頸椎,3例位于胸椎,1例位于腰椎;4例為單中心、2例為多中心病變,3例累及相鄰椎體。CT表現(xiàn):9例均顯示溶骨性骨破壞、周?chē)瞧べ|(zhì)不完整,其中5例腫瘤內(nèi)部可見(jiàn)殘存骨嵴。MRI表現(xiàn):腫瘤于T1WI呈低信號(hào),于T2WI呈混雜信號(hào)(3例)、中等信號(hào)(2例)或高信號(hào)(4例);增強(qiáng)掃描示腫瘤呈明顯強(qiáng)化4例,呈中等強(qiáng)化1例。結(jié)論:惡性血管源性腫瘤主要影像表現(xiàn)為單中心性或多中心溶骨性骨破壞、多位于頸胸椎,部分具有血管瘤的特點(diǎn),容易導(dǎo)致脊柱壓縮變形。
[Abstract]:Objective: to investigate the imaging features of spinal malignant vasogenic tumors. Methods: Ct and MRI findings of 9 cases of spinal malignant vasogenic tumors confirmed by pathology were retrospectively analyzed and their imaging features were summarized. 2 cases of angiosarcoma 7 cases were epithelioid angioendothelioma 5 cases were located in cervical vertebrae, 3 cases were located in thoracic vertebrae, 1 case was located in lumbar vertebra, 4 cases were located in single center, 2 cases were polycentric lesions, 3 cases involved adjacent vertebral bodies, 9 cases showed osteolytic bone destruction, 9 cases showed osteolytic bone destruction. The residual bone crest was found in 5 cases. MRI showed that the tumor showed low signal intensity on T1WI, mixed signal on T2WI in 3 cases, moderate signal intensity in 2 cases) or hyperintense lesions in 4 cases, enhanced obviously in 4 cases on contrast enhanced scan. Conclusion: the main imaging features of malignant angiogenic tumors are monocentric or multi-center osteolytic bone destruction, most of them are located at the cervical and thoracic vertebrae, and some of them have the characteristics of hemangioma, which can easily lead to compression and deformation of the spine.
