【摘要】：目的建立超聲檢測不同孕期胎兒脊髓圓錐末端至最末尾骨遠(yuǎn)端距離的正常值,評估其在脊髓拴系診斷中的價(jià)值。方法研究對象包括400例正常胎兒及8例脊髓拴系的胎兒。隨機(jī)選擇40例胎兒,進(jìn)行醫(yī)師內(nèi)及醫(yī)師間測量結(jié)果的信度分析。建立胎兒脊髓圓錐末端至最末尾骨遠(yuǎn)端距離的正常值范圍,比較正常組及脊髓拴系組的差別。結(jié)果 (1)400例胎兒中,脊髓圓錐末端至最末尾骨遠(yuǎn)端的顯示率為91%。(2)相同及不同醫(yī)師測量的重復(fù)性良好(ICC:0.990~0.998),其95%一致性界限分別為:-2.2~2.6mm和-3.7~3.1mm。(3)胎兒脊髓圓錐末端至最末尾骨遠(yuǎn)端距離與孕周呈線性正相關(guān)關(guān)系,相關(guān)方程為:脊髓圓錐末端至最末尾骨遠(yuǎn)端距離=-23.09+2.39孕周(r=0.965,P0.001)。(4)8例脊髓拴系組的脊髓圓錐末端至最末尾骨遠(yuǎn)端距離與正常組相比明顯縮短(P0.001)。結(jié)論超聲測量胎兒脊髓圓錐末端至最末尾骨遠(yuǎn)端距離簡便可行,這一參數(shù)有助于產(chǎn)前診斷胎兒脊髓拴系。
[Abstract]:Objective to establish a normal value of the distance from the end of the spinal cord cone to the distal end of the tail bone in fetuses during different pregnancy by ultrasound. Methods the study included 400 normal fetuses and 8 cord tethered fetuses. Forty fetuses were randomly selected. To establish the normal range of the distance from the end of the fetal spinal cord cone to the distal end of the tail bone, and to compare the difference between the normal group and the tethered cord group. The display rate of the end of the spinal cone to the distal end of the tail bone was 91. 2) the reproducibility measured by the same and different doctors was good. The 95% consistency limit of ICC: 0. 990 and -3. 710. 6 mm and-3. 731 mm. 3 respectively) the distance from the end of the spinal cord cone to the distal end of the tail bone was positively correlated with the gestational week, and the correlation between the distance between the end of the spinal cord cone and the distal end of the tail bone was positively correlated with the gestational age. The correlation equation is as follows: the distance from the end of the spinal cord cone to the distal end of the tail bone is 23.09 2.39 weeks of gestational age 0.965U 0.965g P 0.001N. The distance from the end of the spinal cord cone to the distal end of the tail bone in the tethered cord group is significantly shorter than that of the normal group. Conclusion Ultrasound measurement of fetal spinal cord circle is a significant reduction of the distance from the distal end of the cone to the distal end of the tail bone compared with the normal group. The distance from the end of the cone to the distal end of the tail bone is simple and feasible. This parameter is helpful for prenatal diagnosis of fetal tethered cord.
