【摘要】：目的對(duì)比分析術(shù)中超聲與神經(jīng)導(dǎo)航在顱內(nèi)腫瘤性病變切除過程中的價(jià)值。方法對(duì)128例(132個(gè)病灶)術(shù)前診斷為顱內(nèi)腫瘤的患者分別于骨瓣打開前后、病灶切除過程中、病灶切除后行術(shù)中超聲及神經(jīng)導(dǎo)航定位探查。結(jié)果在骨瓣打開后132個(gè)腫瘤病灶的術(shù)中超聲均準(zhǔn)確定位,術(shù)中監(jiān)測(cè)病灶殘留程度幾乎與病理及術(shù)后MR結(jié)果相符;神經(jīng)導(dǎo)航在骨瓣打開前均能準(zhǔn)確定位病灶,但骨瓣打開后定位病灶及術(shù)中監(jiān)測(cè)病灶殘留程度部分與病理及術(shù)后MR結(jié)果有偏差。在定位小病灶(≤1cm)及監(jiān)測(cè)膠質(zhì)瘤殘留程度時(shí)術(shù)中超聲較神經(jīng)導(dǎo)航更為準(zhǔn)確,而在定位大病灶(1cm)及監(jiān)測(cè)其他類型病灶切除時(shí)兩者無明顯差異。結(jié)論神經(jīng)導(dǎo)航可在骨瓣打開前準(zhǔn)確定位病灶,但在骨瓣打開后術(shù)中超聲定位更為精確,尤其在定位小病灶(≤1cm)及監(jiān)測(cè)膠質(zhì)瘤殘留程度時(shí)術(shù)中超聲較神經(jīng)導(dǎo)航更有意義。
[Abstract]:Objective to compare the value of intraoperative ultrasound and neuronavigation in the resection of intracranial neoplasms. Results the intraoperative ultrasound localization of 132 tumor lesions after bone flap opening was accurate, and the residual degree of the lesions monitored during the operation was almost consistent with the pathological and postoperative Mr results. Neuronavigation could accurately locate the lesion before the bone flap was opened. However, the localization of lesion after bone flap opening and the monitoring of residual degree of lesion during operation were different from pathological and postoperative Mr results.
