【摘要】：目的:探討三維超聲心動(dòng)圖(RT-3DE)在評(píng)價(jià)房間隔缺損(ASD)封堵術(shù)后右心房功能變化的價(jià)值。方法:納入繼發(fā)孔型ASD患者50例為ASD組,30例體檢正常者為正常對(duì)照組。ASD組的所有患者均成功接受介入封堵術(shù),并分別在術(shù)前、術(shù)后3天、1個(gè)月、3個(gè)月接受RT-3DE檢查。測(cè)量右心房功能相關(guān)指標(biāo):右心房收縮前容積(Vpre)、右心房最小容積(Vmin)、右心房最大容積(Vmax)、右心房總排空容積(Vt)、右心房總排空容積分?jǐn)?shù)(Ft)、主動(dòng)排空容積分?jǐn)?shù)(Fa)、被動(dòng)排空容積分?jǐn)?shù)(Fp)。結(jié)果:封堵術(shù)前,ASD組患者的Vmax、Vmin、Vpre、Vt、Fa均明顯高于正常組(P0.05),而Ft、Fp均低于正常組(P0.05);ASD組患者術(shù)后第3天Vmax、Vmin、Vpre、Vt均高于正常組(P0.05),而Ft、Fp、Fa均低于正常組(P0.05);ASD組患者封堵術(shù)后第3天、1個(gè)月、3個(gè)月的Vmax、Vmin、Vpre、Vt、Fa均明顯低于術(shù)前(P0.05),Ft、Fp均高于術(shù)前(P0.05);ASD組患者術(shù)后第3個(gè)月時(shí)的Vmax、Vmin、Vpre、Vt、Fa、Ft、Fp與正常組比較,差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:RT-3DE顯示ASD患者右心房容積及功能存在異常,封堵術(shù)后,右心房容積及功能在短期內(nèi)即得到改善,但與正常右心房尚有差異;3個(gè)月后,基本恢復(fù)至正常水平。RT-3DE對(duì)評(píng)價(jià)ASD封堵術(shù)后右房容積及功能的變化有重要價(jià)值。
[Abstract]:Objective: to evaluate the value of three dimensional echocardiography RT-3DEE in evaluating the changes of right atrial function after ASD closure. Methods: fifty patients with secondary perforated ASD were included in the ASD group and 30 normal controls. All patients were successfully treated with interventional closure. And before the operation, RT-3DE was performed 3 days, 1 month and 3 months after operation. The relative indexes of right atrial function were measured as follows: right atrial presystolic volume, right atrial minimum volume, right atrium maximal volume, right atrial total emptying volume, right atrial total emptying volume, right atrial total emptying volume, right atrial total emptying volume, right atrial total ejection volume, right atrial presystolic volume, right atrial minimum volume, right atrial maximal volume, right atrial total emptying volume and right atrial total emptying volume. Results: the Vmax Vmin VpreV Fa of ASD group before closure was significantly higher than that of normal group (P 0.05), while FtFP was lower than that of normal group 3 days after operation (P 0.05), and the Vmax Vmin Vmin VpreVT in ASD group was significantly higher than that in normal group 3 days after operation (P 0.05), while FtFpFa Fa Fa Fa was significantly higher in ASD group than that in normal group 3 days after operation. All the patients in the ASD group were lower than those in the normal group on the 3rd day, the first month and the third month after the closure, and the VmaxV minn VpreVt Fa was significantly lower than that in the preoperation P0.05 FtFtFp, which was higher than that in the preoperation P0.05 + ASD group at the 3rd month after the operation, and the VmaxVminn VpreVpreVtTU FtFtFP was significantly higher than that in the normal group at the 3rd month after the operation, compared with the normal group. Conclusion: the volume and function of right atrium in ASD patients were abnormal. After occlusion, the volume and function of right atrium were improved in a short period of time, but still different from those of normal right atrium. Basically returning to normal level. RT-3DE has important value in evaluating the changes of right atrial volume and function after ASD closure.
