【摘要】：目的:分析不同病理類型的孤立性肺小結(jié)節(jié)(SPN)的胸部CT表現(xiàn)(大小、生長部位及形態(tài)學(xué)特征),探討其對SPN定性診斷的價(jià)值。方法:回顧性分析經(jīng)病理證實(shí)的94例SPN患者行多層螺旋CT掃描后處理的圖像,觀察結(jié)節(jié)大小、分布、邊緣及內(nèi)部征象并與病理結(jié)果對照。結(jié)果:94例SPN中,良性結(jié)節(jié)26例(占27.7%),其中包括炎性結(jié)節(jié)13例(13.8%),結(jié)核瘤5例(5.3%),錯(cuò)構(gòu)瘤7例(7.4%),硬化性血管瘤1例(1.1%)。惡性結(jié)節(jié)68例(占72.3%),其中包括腺癌51例(54.3%),鱗癌11例(11.7%),腺鱗癌1例(1.1%),小細(xì)胞癌2例(2.1%),大細(xì)胞癌1例(1.1%),類癌2例(2.1%)。良性結(jié)節(jié)平均直徑(1.87±0.60)cm,惡性結(jié)節(jié)平均直徑(2.09±0.67)cm。所有病例中左肺結(jié)節(jié)共46例,其中惡性33例,良性13例;右肺結(jié)節(jié)48例,其中惡性35例,良性13例。惡性結(jié)節(jié)在肺葉的分布總體差異無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)節(jié)的分葉征、毛刺征、胸膜凹陷征、血管集束征的發(fā)生率在惡性組明顯高于良性組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:對SPN患者早期行CT檢查,分析其邊緣及內(nèi)部形態(tài)特征,可作出對SPN的良惡性鑒別診斷,對臨床治療具有重要的指導(dǎo)價(jià)值。
[Abstract]:Objective: to analyze the chest CT findings (size) of solitary pulmonary nodules (SPNs) with different pathological types. To explore the value of growth site and morphological features in the qualitative diagnosis of SPN methods: the image of 94 cases of SPN confirmed by pathology were analyzed retrospectively and the size and distribution of the nodules were observed. The marginal and internal signs were compared with pathological findings. Results in 94 cases of SPN, There were 26 benign nodules (27.7%), including 13 cases of inflammatory nodules, 5 cases of tuberculoma, 7 cases of hamartoma, 1 case of sclerosing hemangioma, 1 case of sclerosing hemangioma, 68 cases of malignant nodules, including 51 cases of adenocarcinoma, 11 cases of squamous cell carcinoma, 11 cases of squamous cell carcinoma and 1 case of adenosquamous carcinoma. The mean diameters of benign and malignant nodules were 1.87 鹵0.60 cm and 2.09 鹵0.67 cm respectively. There were 46 cases of left pulmonary nodules in all cases. Among them, 33 cases were malignant, 13 cases were benign, 48 cases were right pulmonary nodule, of which 35 cases were malignant, 13 cases were benign. There was no significant difference in the distribution of malignant nodules in pulmonary lobes (P 0.05). The incidence of vascular cluster sign in malignant group was significantly higher than that in benign group, and the difference was statistically significant (P 0.05). Conclusion: early CT examination and analysis of the edge and internal morphological features of SPN patients can make differential diagnosis of benign and malignant SPN. It has important guiding value for clinical treatment.
