【摘要】：目的:利用彩色多普勒超聲心動圖對小兒繼發(fā)孔型房間隔缺損與卵圓孔未閉的自然愈合情況進(jìn)行隨訪,比較兩者在自然愈合時(shí)間和愈合率上的差別。方法:依據(jù)房間隔缺損和卵圓孔未閉的判斷標(biāo)準(zhǔn)對存在心房間分流的220例新生兒進(jìn)行超聲心動圖檢查,并將其分成三組:房間隔缺損直徑≤3mm組(A組)48例、房間隔缺損直徑3mm組(B組)30例,卵圓孔未閉組(C組)142例,分別對其在出生后6個月、出生后1年、出生后2年進(jìn)行定期超聲隨訪,記錄各組的自然愈合例數(shù)并計(jì)算出自然愈合率。結(jié)果:A組出生后6個月、出生后1年和出生后2年的自然愈合率分別為16.7%(8/48)、52.1%(25/48)、93.8%(45/48);B組分別為:6.7%(2/30)、13.3%(4/30)、23.3%(7/30);C組分別為:80.3%(114/142)、95.1%(135/142)、98.6%(140/142)。對同一時(shí)間點(diǎn)上的三組的自然愈合率兩兩進(jìn)行卡方檢驗(yàn),出生后6個月時(shí),C組的自然愈合率高于A組和B組(P0.05),A組和B組比較無統(tǒng)計(jì)學(xué)意義(P0.05),出生后1年時(shí),C組高于A組和B組(P0.05),A組高于B組(P0.05),出生后2年時(shí),C組和A組均高于B組(P0.05),A組和C組比較無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:繼發(fā)孔型房間隔缺損在出生后6個月內(nèi)不容易愈合,當(dāng)繼發(fā)型房間隔缺損的直徑≤3mm時(shí),其出生后6個月至2年時(shí)自然愈合的概率很高,而當(dāng)缺損的直徑3mm則不容易自然愈合,卵圓孔未閉大多在6個月至1歲時(shí)自然愈合,相對房間隔缺損要早一些,然而出生后2年時(shí)它與3mm的繼發(fā)孔型房間隔缺損的自然愈合率差別不大。
[Abstract]:Objective: to follow up the natural healing of secondary atrial septal defect (ASD) and patent foramen ovale in children by color Doppler echocardiography. Methods: according to the criteria of atrial septal defect and patent foramen ovale, 220 neonates with cardiac shunt were examined by echocardiography. They were divided into three groups: group A (n = 48), group A (n = 48), group B (n = 30) with diameter of atrial septal defect (n = 3), group C (n = 142) with patent foramen ovale. Two years after birth, regular ultrasound follow-up was performed to record the number of cases of natural healing in each group and to calculate the rate of natural healing. The natural healing rates in the first year after birth and in the second year after birth were 16.780 / 48, 52.1Q / 25 / 93.885 and 454548m / b, respectively. The natural healing rate in group B was 6.7R / 2.3030 / 13.3C = 80.33.30 / 30C = 80.3114142/ 95.998 / 698.698.698.698.698.698.698.698.698.698 / 142Q / 1421.The natural healing rate of the three groups at the same time was examined by caloric test. At 6 months after birth, the rate of spontaneous healing in group C was higher than that in group A and group B (P 0.05). There was no significant difference between group A and group B, but at one year after birth, the rate of spontaneous healing in group C was higher than that in group A and group B (P 0.05), and in group C and group A were higher than that in group B at 2 years after birth.
