【摘要】：目的探討小腸淋巴瘤的CT和MRI表現(xiàn)及其病理相關(guān)性。方法回顧性分析28例經(jīng)病理證實(shí)的小腸淋巴瘤患者的CT和MRI資料。結(jié)果 28例均為非霍奇金淋巴瘤,B細(xì)胞淋巴瘤24例,T細(xì)胞淋巴瘤4例;共發(fā)現(xiàn)非連續(xù)病灶41個,腸壁浸潤型病灶30個,腸系膜腫塊型7個,洞穴型3個,息肉腫塊型1個。6例病變腸段管腔呈動脈瘤樣擴(kuò)張。20例伴腹腔淋巴結(jié)腫大,明顯包繞腸系膜血管呈"夾心面包征"8例。其他器官受累共7例。14例彌漫大B細(xì)胞淋巴瘤中12例發(fā)生于回腸,4例T細(xì)胞淋巴瘤中3例發(fā)生于空腸,1例發(fā)生于十二指腸;2例T細(xì)胞淋巴瘤為浸潤型,其腸壁增厚均不明顯;14例彌漫大B細(xì)胞淋巴瘤均未見穿孔;3例T細(xì)胞淋巴瘤淋巴結(jié)腫大不顯著。結(jié)論均勻較長節(jié)段的腸壁增厚、腸腔呈動脈瘤樣擴(kuò)張、明顯的腸系膜淋巴結(jié)腫大并包繞腸系膜血管呈"夾心面包征"的典型征象有助于小腸淋巴瘤的正確診斷。
[Abstract]:Objective to investigate CT and MRI manifestations of small intestinal lymphoma and its pathological correlation. Methods Retrospective analysis of 28 cases of CT and MRI data of patients with small intestinal lymphoma confirmed by pathology. Results 28 cases were non Hodgkin lymphoma, 24 cases of B cell lymphoma, 4 cases of T cell lymphoma; non continuous were found in 41 lesions, bowel wall invasion type 30 lesions, mesenteric mass type 7, cave type 3, type 1.6 cases of polyp mass lesions of the bowel lumen showed aneurysmal dilatation of.20 patients with abdominal lymph node enlargement, was wrapped around the mesenteric vessels were "sandwich" sign in 8 cases. The other organ involved a total of 7 cases of.14 patients with diffuse large B cell lymphoma in 12 cases occurred in the ileum, 4 cases of T cell lymphoma in 3 cases occurred in the jejunum, 1 cases occurred in the duodenum; 2 cases of T cell lymphoma infiltrating type, the bowel wall thickening was not obvious; 14 cases of diffuse large B cell lymphoma were no perforation; 3 cases of T cell lymphoma Lymph node enlargement was not significant. Conclusion the uniform thickening of the intestinal wall long segments of the intestinal lumen showed aneurysmal dilatation, mesenteric lymph node enlargement of mesenteric vascular encasement of typical signs of a "sandwich" sign is helpful to the correct diagnosis of small intestinal lymphoma significantly.
