【摘要】：目的探討脾臟單發(fā)腫瘤或腫瘤樣病變的CT表現(xiàn)。方法回顧性分析18例經(jīng)手術(shù)病理證實(shí)的脾臟單發(fā)腫瘤或腫瘤樣病變的臨床表現(xiàn)及CT特點(diǎn)。結(jié)果 18例單發(fā)脾臟腫瘤或腫瘤樣病變中,血管瘤2例,其中1例明顯強(qiáng)化,1例邊緣結(jié)節(jié)狀強(qiáng)化;3例淋巴管瘤邊緣及分隔輕度強(qiáng)化;3例較大竇岸細(xì)胞血管瘤動(dòng)脈期明顯強(qiáng)化,門靜脈期逐漸均勻強(qiáng)化,延遲期強(qiáng)化與周圍組織相仿,另2例邊緣輕度強(qiáng)化;3例較大硬化性血管瘤結(jié)節(jié)樣轉(zhuǎn)化病灶內(nèi)見"輻輪狀鈣化"及"輻輪狀強(qiáng)化",另1例病灶內(nèi)見液化壞死;1例上皮樣血管內(nèi)皮瘤平掃及增強(qiáng)無特征性改變;1例巨塊型淋巴瘤動(dòng)脈期可見"血管漂浮"征;2例淋巴瘤呈輕度均勻強(qiáng)化。結(jié)論 CT可較清晰地顯示脾臟病灶的大小、形狀、密度、邊界及強(qiáng)化程度,對(duì)脾臟單發(fā)腫瘤或腫瘤性病變有較高的診斷價(jià)值。
[Abstract]:Objective to investigate the CT features of splenic single tumor or tumor-like lesions. Methods the clinical manifestations and CT features of 18 cases of splenic single tumor or tumor-like lesions confirmed by surgery and pathology were retrospectively analyzed. In tumors or tumor-like lesions, In 2 cases of hemangioma, 1 case was obviously enhanced with marginal nodular enhancement of lymphangioma, 3 cases with lymphangioma and 3 cases with large sinus cell hemangioma during arterial phase, portal vein phase was gradually enhanced evenly. The delayed phase enhancement was similar to that of surrounding tissues. In the other 2 cases, slight enhancement of the margin was seen in 3 cases of nodular transformation lesions of larger sclerosing hemangioma with "radiocalcification" and "radionuclide enhancement". In the other case, liquefaction necrosis was found in 1 case of epithelioid angioendothelioma without plain scan and enhancement. In 1 case of giant mass lymphoma, "vessel floating" sign was observed in arterial phase. Conclusion CT can clearly show the size of splenic lesions. Shape, density, boundary and enhancement are valuable for the diagnosis of single tumor or tumorous lesion of spleen.
