【摘要】：目的:探討經會陰超聲檢查在女性壓力性尿失禁(Stress urinary incontinence,SUI)中的應用價值。方法:對2013年8月—2014年3月河北聯合大學附屬醫(yī)院收治的79例SUI患者(輕度SUI組40例、中重度SUI組39例),同期因健康查體就診后排除SUI和(或)盆腔器官脫垂的患者41例(對照組)行經會陰超聲檢查,在安靜狀態(tài)下測量膀胱尿道后角(α),在屏氣用力動作時檢測患者膀胱頸移動度(Mu),將兩種狀態(tài)下的圖像凍結并存入計算機,之后測量膀胱頸旋轉角度(θ)、膀胱頸下移距離(X)。結果:對照組與輕度SUI組、中重度SUI組間的Mu值、α角、θ角差異均有統(tǒng)計學意義(P0.05),且與SUI的嚴重程度呈正相關(Mu、α、θ的r值分別為0.861,0.750,0.619,P0.05)。對照組與輕度SUI組、中重度SUI組間的X值差異有統(tǒng)計學意義(P0.05),而輕度SUI組與中重度SUI組間的X值差異無統(tǒng)計學意義(P0.05)。結論:經會陰超聲檢查能夠清晰、直觀地顯示盆底解剖結構及功能改變,在SUI的診斷中具有重要的應用價值。
[Abstract]:Objective: to investigate the value of transperineal ultrasonography in the diagnosis of stress urinary incontinence in female patients with stress urinary incontinence. Methods: from August 2013 to March 2014, 79 patients with SUI (mild SUI group, 40 cases) were treated in affiliated Hospital of Hebei Union University. 39 cases of moderate and severe SUI group were treated with transperineal ultrasonography. 41 patients who excluded SUI and / or pelvic organ prolapse after health examination were examined by transperineal ultrasonography. The posterior angle of bladder urethra (偽) was measured in a quiet state. The bladder neck mobility was measured during the breath holding force, and the images in both states were frozen and stored in a computer. Then the rotation angle of bladder neck (胃) was measured. Results: control group and mild SUI group. There were significant differences in Mu value, 偽 angle and 胃 angle between moderate and severe SUI groups (P 0.05), and the r values of Mu, 偽, 胃 were 0.861C 0.7500.619 (P0.05), respectively. There was significant difference between the control group and the mild SUI group (P 0.05), and there was a positive correlation between Mu, 偽, 胃 and the severity of SUI in the control group and the mild SUI group. There was significant difference in X value between moderate and severe SUI group, but there was no significant difference in X value between mild SUI group and moderate and severe SUI group. Conclusion: transperineal ultrasonography can clearly and intuitively display the changes of pelvic floor anatomical structure and function. It has important application value in the diagnosis of SUI.
