【摘要】：目的:探討肺腺癌純磨玻璃密度結(jié)節(jié)(pure ground-glass opacity,p GGO)內(nèi)部CT影像學(xué)特征。方法:回顧性分析156個(gè)手術(shù)病理證實(shí)的p GGO結(jié)節(jié)內(nèi)部CT影像特點(diǎn)。所有p GGO結(jié)節(jié)中浸潤前病變58個(gè)(包括不典型腺瘤樣增生及原位腺癌)、微浸潤腺癌32個(gè)和浸潤性腺癌66個(gè)。p GGO結(jié)節(jié)內(nèi)部CT影像特征包括病變密度均勻/不均勻、空泡征、空氣支氣管征及病變內(nèi)走行血管異常改變(血管擴(kuò)張、扭曲、僵直)。結(jié)果:p GGO結(jié)節(jié)中密度不均、病變內(nèi)血管異常改變在3種不同病理分型中差異有統(tǒng)計(jì)學(xué)意義(χ2=7.452,P=0.024;(χ2=22.615,P0.001),隨著結(jié)節(jié)浸潤程度的增加,病變密度不均及其內(nèi)血管異常改變的發(fā)生率隨之增加;空氣支氣管征的發(fā)生率亦隨病變浸潤程度的加深而增高,但其僅對于浸潤前病變與浸潤性病變(包括微浸潤腺癌和浸潤性腺癌)的鑒別診斷有價(jià)值(χ~2=4.868,P=0.027)。結(jié)論:p GGO結(jié)節(jié)內(nèi)部密度不均、血管異常改變對肺腺癌不同病理類型的鑒別具有一定的價(jià)值。
[Abstract]:Objective: to investigate the internal CT imaging features of pure ground-glass opacities in lung adenocarcinoma. Methods: the internal CT features of 156 surgically and pathologically proved p GGO nodules were retrospectively analyzed. All preinvasive lesions in all p GGO nodules were analyzed retrospectively. In 58 cases (including atypical adenomatous hyperplasia and in situ adenocarcinoma, 32 microinvasive adenocarcinomas and 66. P GGO nodules), CT features included homogeneous or uneven density of lesions. Vacuole sign, air bronchus sign and abnormal changes of blood vessel (vasodilation, distortion, rigidity) were observed in the lesions. Results the density of GGO nodules was uneven. There were significant differences among the three pathological types in the abnormal changes of blood vessels in the lesions (蠂 ~ 2 ~ 2 ~ (7.452) P ~ (0.024)) (蠂 ~ (2) 2 ~ (2) 22.615 ~ (15) P _ (0.001)). With the increase of the degree of nodular infiltration, the incidence of abnormal changes in the density of lesions and the abnormal changes of the internal vessels increased. The incidence of air bronchiolus sign also increased with the degree of infiltration. However, it is of value only for the differential diagnosis of preinvasive lesions and invasive lesions (including microinvasive adenocarcinoma and invasive adenocarcinoma) (蠂 ~ 2 / P = 4.868). Conclusion the density of GGO nodules is uneven. The abnormal changes of blood vessels have certain value in differentiating different pathological types of lung adenocarcinoma.
