【摘要】：目的采用二維斑點(diǎn)追蹤技術(shù)(STE)評(píng)價(jià)急性心肌梗死(AMI)患者不同冠脈病變部位對(duì)左心房功能的影響。方法根據(jù)冠狀動(dòng)脈造影顯示的犯罪血管將患者分為L(zhǎng)AD組,LCX組及RCA組,其中LAD組31例,LCX組22例,RCA組26例。分別在PCI術(shù)后72h內(nèi)行超聲檢查,檢測(cè)左心房整體心肌收縮期縱向應(yīng)變(LS)和應(yīng)變率(LSR-s)、舒張?jiān)缙趹?yīng)變率(LSR-e)及舒張晚期應(yīng)變率(LSR-a)。PCI再通1個(gè)月后隨訪。結(jié)果 LCX組左心房LS及LSR-s較LAD組及RCA組明顯減低(P0.05)。三組左心室舒張功能、左心房容積及射血分?jǐn)?shù)無(wú)顯著差異。PCI再通1個(gè)月后,三組左心房功能無(wú)顯著差異。結(jié)論 LCX組閉塞患者左心房功能較LAD組及RCA組閉塞患者更加減低,PCI再通后,左心房功能恢復(fù)。
[Abstract]:Objective to evaluate the effects of different coronary lesions on left atrial function in patients with acute myocardial infarction (AMI) by using two dimensional speckle tracing technique. Methods according to the criminal vessels displayed by coronary angiography, the patients were divided into LAD group and RCA group. Among them, 31 cases in LAD group and 22 cases in RCA group were examined by ultrasound within 72 hours after PCI. The left atrial systolic longitudinal strain (LSs), strain rate (LSR-S), early diastolic strain rate (LSR-e) and late diastolic strain rate (LSR-aI) and late diastolic strain rate (LSR-A 路PCI) were measured. Results the left atrial LS and LSR-s in LCX group were significantly lower than those in LAD group and RCA group. Left ventricular diastolic function, There was no significant difference in left atrial volume and ejection fraction. There was no significant difference in left atrial function among the three groups one month after PCI recanalization. Conclusion the left atrial function in LCX group is significantly lower than that in LAD group and RCA group.
