【摘要】：目的探討超微血管成像(SMI)技術(shù)在鑒別乳腺腫物良惡性腫瘤中的應(yīng)用價(jià)值。方法選取超聲檢出并術(shù)后病理證實(shí)的乳腺腫瘤患者70例78個(gè)病灶,其中良性病灶40個(gè),惡性病灶38個(gè)。按照Adler分級(jí)標(biāo)準(zhǔn),采用CDFI和SMI技術(shù)分別觀察同一腫瘤的血流情況,計(jì)算兩種血流檢測(cè)技術(shù)對(duì)于單個(gè)病灶檢出血管條數(shù)的差值(SMI-CDFI)、單純使用CDFI及SMI診斷惡性病變的最佳診斷閾值及相關(guān)診斷試驗(yàn)評(píng)價(jià)指標(biāo)。結(jié)果兩種檢測(cè)方法對(duì)所有腫瘤血流豐富程度的檢測(cè)差異有統(tǒng)計(jì)學(xué)意義(P0.05);對(duì)惡性腫瘤血流豐富程度的檢測(cè)差異有統(tǒng)計(jì)學(xué)意義(P0.01);而對(duì)良性腫瘤血流豐富程度的檢測(cè)差異無(wú)統(tǒng)計(jì)學(xué)意義(P=0.14)。CDFI診斷惡性腫物最佳診斷閾值為血管條數(shù)≥2,敏感度71.12%,特異度62.50%;SMI診斷惡性腫物最佳診斷閾值為血管條數(shù)≥3,敏感度92.11%,特異度62.50%;SMI-CDFI診斷惡性腫物最佳診斷閾值為血管條數(shù)差值≥3,敏感度84.21%,特異度85.00%。結(jié)論 SMI技術(shù)可檢測(cè)乳腺惡性腫物潛在的微血管,可為鑒別乳腺腫瘤的良惡性提供可靠的參考依據(jù),有廣泛的臨床應(yīng)用前景。
[Abstract]:Objective to evaluate the value of supermicrovascular imaging (SMI) in differentiating benign and malignant breast tumors. Methods 70 patients with breast tumors diagnosed by ultrasonography and confirmed by pathology after operation were selected, including 40 benign lesions. According to Adler grading standard, the blood flow of the same tumor was observed by CDFI and SMI. To calculate the difference of the number of blood vessels detected by two blood flow detection techniques and the optimal diagnostic threshold of CDFI and SMI for the diagnosis of malignant lesions and to evaluate the relevant diagnostic criteria. Results the two methods were applied to all lesions. There was significant difference in blood flow richness of tumor (P 0.05), malignant tumor (P 0.01), and benign tumor (P 0. 14). CDFI was not significant in the diagnosis of malignant malignancy (P < 0. 05), but there was no significant difference in the detection of blood flow richness (P = 0. 14.
