【摘要】：目的探討CEUS對甲狀腺TI-RADS 3、4級結節(jié)的診斷價值。方法回顧性分析95例常規(guī)超聲診斷為TIRADS 3、4級的甲狀腺結節(jié)(共95個結節(jié),均經手術病理證實)的CEUS表現(xiàn),分析CEUS對甲狀腺TI-RADS 3、4級結節(jié)的校正及鑒別診斷價值。結果常規(guī)超聲TI-RADS分級評估甲狀腺結節(jié)良惡性與手術病理級型比較,差異無統(tǒng)計學意義(χ~2=3.56,P=0.06)。對于直徑1cm的甲狀腺TI-RADS 3、4級結節(jié),與常規(guī)超聲TI-RADS分級相比,CEUS評分及CEUS校正后TI-RADS分級的診斷準確率差異均有統(tǒng)計學意義(P=0.03、0.01);對于直徑≤1cm的微小乳頭狀癌,差異均無統(tǒng)計學意義(P=0.25、1.00)。ROC曲線分析顯示,對于甲狀腺TI-RADS 3、4級結節(jié),常規(guī)超聲TI-RADS分級、CEUS評分、CEUS校正后TI-RADS分級的ROC曲線下面積分別為0.64、0.75、0.81時,截斷值分別為TI-RADS 4a級、1分、TI-RADS 4a級,評估結節(jié)良、惡性的敏感度、特異度分別為45.3%、80.0%,69.3%、65.0%,82.7%、60.0%。CEUS評分、CEUS校正后TI-RADS分級與常規(guī)超聲TI-RADS分級的曲線下面積的差異有統(tǒng)計學意義(P均0.05),CEUS評分與CEUS校正后TI-RADS分級的曲線下面積差異無統(tǒng)計學意義(P=0.23)。結論 CEUS對甲狀腺TIRADS 3、4級結節(jié)具有校正及提高鑒別診斷的價值。
[Abstract]:Objective to evaluate the value of CEUS in the diagnosis of thyroid TI-RADS 3 grade 4 nodule. Methods the CEUS findings of 95 cases of thyroid nodule with grade 3 of TIRADS 3 or grade 4 diagnosed by conventional ultrasound were retrospectively analyzed. The value of CEUS in the correction and differential diagnosis of thyroid TI-RADS 3 or 4 grade nodule was analyzed. Results there was no significant difference between benign and malignant thyroid nodules by conventional ultrasound TI-RADS grading (蠂 2 / 3.56) and thyroid TI-RADS grade 3 / 4 nodule with diameter of 1 cm (蠂 2 / 3.56). Compared with the conventional TI-RADS grade, the diagnostic accuracy of CEUs and TI-RADS after CEUS correction were significantly higher than that of conventional ultrasound, and there was no significant difference in the diagnosis accuracy of P0. 03 or 0. 01% for small papillary carcinoma with diameter 鈮，
