彌散張量成像軀體化障礙腦微結構研究

【摘要】:目的:運用磁共振彌散張量成像定量指標測定軀體化障礙(SD)患者腦內超微結構的變化,以期對軀體化障礙的發(fā)病機制、臨床研究提供有價值的依據(jù)。方法:采用3.0T磁共振對28例SD患者及28例正常對照進行DTI掃描,測量受試者左右側額葉白質纖維、胼胝體、左右側扣帶束、左右上、下縱束感興趣區(qū)內主要白質纖維束以及左右側小腦齒狀核的表觀彌散系數(shù)(ADC)和各向異性分數(shù)(FA)值,并進行兩組間比較,評估其在病例組和對照組之間的差異,分析兩組間FA值有統(tǒng)計學差異的腦區(qū)的FA值與病程的相關性。結果:患者組胼胝體膝部FA值較對照組降低,左側下縱束、左右側扣帶束水平部、左扣帶束后部的ADC值較對照組明顯升高,FA值較對照組明顯降低(P0.05),有差異腦區(qū)的FA值與病程之間無統(tǒng)計學相關性。SD組右側小腦齒狀核FA值較左側升高,二者之間差異有統(tǒng)計學意義,正常組左右側小腦髓質和齒狀核、正常和病例組間相應小腦髓質和齒狀核ADC、FA值比較無統(tǒng)計學差異。結論:軀體化障礙患者優(yōu)勢半球扣帶束、下縱束神經纖維完整性降低,存在超微結構異常,SD患者雙側小腦齒狀核存在明顯不對稱性,可能是軀體化障礙患者病理生理基礎之一,但是病程與神經纖維損傷程度未發(fā)現(xiàn)明顯相關性。
[Abstract]:Objective: to determine the changes of brain ultrastructure in patients with somatization disorder (SDD) by using the quantitative index of magnetic resonance diffusion Zhang Liang imaging, in order to explore the pathogenesis of somatization disorder. Methods: DTI scanning was performed on 28 SD patients and 28 normal controls with 3.0T magnetic resonance imaging. The white matter fibers of left and right frontal lobe, corpus callosum, cingulate band of left and right were measured. The apparent diffusion coefficient (ADCC) and anisotropic fraction (FAA) of the main white matter fiber bundle and the left and right cerebellar dentate nuclei in the region of interest of the lower longitudinal bundle were compared and compared between the two groups to evaluate the difference between the case group and the control group. Results: the FA value of the genu of corpus callosum in the patients group was lower than that of the control group. The ADC value of the posterior part of the left cingulate band was significantly higher than that of the control group. The FA value of the right cerebellar dentate nucleus in SD group was higher than that of the left cerebellar dentate nucleus. There was no significant difference in ADCFA between the left and right cerebellum medulla and dentate nucleus in normal group, and the corresponding ADCFA value in cerebellar medulla and dentate nucleus between normal group and case group. Conclusion: the dominant hemispheric cingulate bundle is found in patients with somatization disorder. The nerve fiber integrity of the lower longitudinal bundle decreased and the bilateral dentate nucleus of the cerebellum showed obvious asymmetry in the patients with abnormal ultrastructure, which may be one of the pathophysiological bases of the patients with somatization disorder. However, no significant correlation was found between the course of disease and the degree of nerve fiber injury.
