【摘要】：MRI憑借多方位成像和良好的組織對(duì)比度,在評(píng)價(jià)卵巢腫瘤的良惡性時(shí)具有重要的診斷價(jià)值。然而,鑒別原發(fā)卵巢惡性腫瘤(POMT)和轉(zhuǎn)移瘤(MOT)的特異性MRI征象尚未見(jiàn)報(bào)道。MOT占所有卵巢惡性腫瘤的10%~30%,與POMT在影像及病理上非常相似,兩者的鑒別很困難。不同原發(fā)腫瘤的MOT有各自特征,如K r ukenberg瘤典型表現(xiàn)為雙側(cè)、實(shí)性、T2WI上非均質(zhì)信號(hào)的腫塊;結(jié)腸癌的MOT常為雙側(cè)、伴不同大小實(shí)性成分的囊性腫瘤;源自闌尾的MOT常為雙側(cè)卵巢黏液性腫瘤,伴腹膜假性黏液瘤。近年來(lái)功能成像如擴(kuò)散加權(quán)成像(DWI)、灌注加權(quán)成像(PWI)及磁共振波譜(MRS)的發(fā)展和應(yīng)用,使MRI在卵巢腫瘤的鑒別診斷、預(yù)后及療效評(píng)價(jià)等方面發(fā)揮越來(lái)越重要的作用。
[Abstract]:MRI has important diagnostic value in the evaluation of benign and malignant ovarian tumors by virtue of multidirectional imaging and good tissue contrast. The specific MRI signs of differential diagnosis of primary ovarian malignant tumor (MRI) and metastatic tumor (mot) have not been reported. MOT accounts for 10% of all ovarian malignant tumors, which is very similar to POMT in imaging and pathology, so it is very difficult to differentiate between them. The MOT of different primary tumors has its own characteristics. For example, the typical features of K r ukenberg tumor are bilateral, heterogeneous mass on solid T _ 2WI; MOT of colon cancer is usually bilateral, with different size solid components of cystic tumor; MOT originating from appendix is usually bilateral ovarian mucinous tumor. In recent years, the development and application of functional imaging, such as diffusion weighted imaging (DWI), perfusion weighted imaging (PWI) and magnetic resonance spectroscopy (MRS), in the differential diagnosis of ovarian tumors. Prognosis and evaluation of curative effect play a more and more important role.
