高膽紅素血癥新生兒顱腦MRI特征與血清肝功能指標(biāo)的相關(guān)性

【摘要】:目的探討高膽紅素血癥新生兒顱腦6個部位的常規(guī)MRI信號強度、表觀擴散系數(shù)(ADC)值與肝功能血清學(xué)指標(biāo)的相關(guān)性。方法搜集臨床診斷為高膽紅素血癥的60例新生兒治療前的MRI資料;仡櫺苑治銎渖n白球、殼核、海馬、黑質(zhì)、背側(cè)丘腦、底丘腦的T1WI信號強度、T2WI信號強度及ADC值,并與血清谷草轉(zhuǎn)氨酶(AST)、血清總膽紅素(TBIL)、血清間接膽紅素(IBIL)對照。結(jié)果 60例高膽紅素血癥患兒蒼白球、殼核、海馬、黑質(zhì)、背側(cè)丘腦、底丘腦的T1WI信號強度均值分別為909.65±329.33、781.42±272.72、725.33±261.90、1009.67±378.04、867.81±875.50、905.43±391.74;T2WI信號強度均值分別為1247.68±116.79、1238.34±126.32、1271.32±105.33、1168.79±131.38、1242.41±124.21、1235.47±148.17;ADC均值(×10-3mm2/s)分別為1134.28±123.79、1119.49±205.57、1148.67±79.69、1040.46±108.41、1075.22±194.09、1257.77±1447.75。6個部位間2種信號強度的差異有統(tǒng)計學(xué)意義(F=2.297,P=0.045;F=3.733,P=0.003),ADC值的差異無統(tǒng)計學(xué)意義(F=0.725,P=0.605)。背側(cè)丘腦T2WI信號強度(1242.41±124.21)與AST(61.99±35.40)呈負(fù)相關(guān);底丘腦T1WI信號強度(905.43±391.74)與經(jīng)皮膚測量顱腦額部、胸部的膽紅素濃度(12.36±5.08、12.80±5.49)均呈正相關(guān)。結(jié)論 MRI指標(biāo)與血清肝功能相關(guān)指標(biāo)存在相關(guān)性,MRI檢查有望為臨床早期、無創(chuàng)的判斷高膽紅素血癥對新生兒腦損傷的程度提供更多客觀評價方法。
[Abstract]:Objective to investigate the conventional MRI signal intensity in 6 brain regions of neonates with hyperbilirubinemia. Methods the MRI data of 60 newborns diagnosed as hyperbilirubinemia before treatment were collected and analyzed retrospectively, including globus pallidus, putamen putamen, hippocampus, substantia nigra, dorsal thalamus. T1WI signal intensity of subthalamus and T2WI signal intensity and ADC were compared with serum glutamic oxaloacetic transaminase (AST), serum total bilirubin (TBILL) and indirect bilirubin (IBIL). Results 60 cases of hyperbilirubinemia were treated with globus pallidus, putamen putamen, hippocampus, substantia nigra, dorsal thalamus. The mean T1WI signal intensity of the subthalamus on T 1WI was 909.65 鹵329.333781.42 鹵272.72 725.33 鹵261.90 1009.67 鹵378.0447 鹵867.81 鹵875.50905.43 鹵391.74 / T _ 2WI respectively: 1247.68 鹵116.791238.34 鹵126.3292.32 鹵105.331168.79 鹵131.31242.41 鹵124.211242.41 鹵124.2112447 鹵148.17a ADC (脳 10-3mm ~ 2s) respectively was 1134.28 鹵123.7919.49 鹵205.5m-2 / s 71148.67 鹵79.91040.46 鹵108.48.104.104.77.74.74.74.74.77.77.77.74.77.74.77.72.77.72.72.72.42 鹵124.21242.41 鹵124.2128.447 鹵148.77.77.72.77.72 respectively. The intensity of 1242.41 鹵124.21) was negatively correlated with AST(61.99 鹵35.40). The signal intensity of T1WI in the subthalamus was positively correlated with the bilirubin concentration in the frontal part of the brain and chest measured by skin. Conclusion there is a positive correlation between MRI and serum liver function. Non-invasive assessment of hyperbilirubinemia for neonatal brain injury provides more objective evaluation methods.
