心臟磁共振預(yù)測特發(fā)性肺動脈高壓患者右心室收縮壓的可行性研究

【摘要】:目的評價(jià)使用CAD畫圖軟件對心臟磁共振圖像進(jìn)行測量并計(jì)算室間隔和左心室游離壁的曲率比來預(yù)測特發(fā)性肺動脈高壓患者右心室收縮壓的準(zhǔn)確性和可重復(fù)性。方法由兩名醫(yī)師利用第三方CAD畫圖軟件測量、計(jì)算20例確診為特發(fā)性肺動脈高壓患者心臟磁共振收縮末期圖像室間隔與左心室游離壁的曲率比。BlandAltman分析評估室間隔與左心室游離壁曲率比測量計(jì)算的可重復(fù)性。將CAD軟件測量、計(jì)算的室間隔與左心室游離壁曲率比和右心導(dǎo)管檢查測量、計(jì)算的室間隔與左心室游離壁跨壁壓力差比進(jìn)行線性回歸分析得到回歸方程,通過此方程計(jì)算出右心室收縮壓。Bland-Altman分析評估計(jì)算得到的和右心導(dǎo)管測得的兩組右心室收縮壓的一致性。結(jié)果室間隔與左心室游離壁曲率比和室間隔與左心室游離壁跨壁壓力差比呈顯著線性相關(guān)(r=0.93)。經(jīng)室間隔與左心室游離壁的曲率比計(jì)算得到的和右心導(dǎo)管測得的兩組右心室收縮壓一致性較好[平均差異(7.8±12.1)mm Hg]。結(jié)論使用CAD畫圖軟件測量、計(jì)算心臟磁共振圖像室間隔與左心室游離壁的曲率比簡便、快速、可重復(fù)性好,測量得出的室間隔與左心室游離壁的曲率比可以較準(zhǔn)確地預(yù)測特發(fā)性肺動脈高壓患者的右心室收縮壓。
[Abstract]:Objective to evaluate the accuracy and reproducibility of the right ventricular systolic pressure (RVSBP) prediction in patients with idiopathic pulmonary hypertension by using CAD software to measure cardiac magnetic resonance images and calculate the curvature ratio of ventricular septum and left ventricular free wall. The method was measured by two physicians using third-party CAD drawing software, To calculate the curvature ratio of ventricular septum to left ventricular free wall in 20 patients diagnosed as idiopathic pulmonary hypertension. Bland Altman analysis was used to evaluate the repeatability of calculating the curvature ratio of ventricular septum to left ventricular free wall. CAD software was used to measure the ratio of ventricular septum to left ventricular free wall curvature. The calculated ratio of free wall curvature between the ventricular septum and the left ventricle and the right ventricular catheterization were measured. The regression equation was obtained by linear regression analysis of the calculated ratio of pressure difference between the ventricular septum and the left ventricular free wall. The right ventricular systolic pressure calculated by Bland-Altman analysis and the right ventricular systolic pressure measured by right ventricular catheterization were consistent. Results the ratio of curvature of septal to left ventricular free wall and the ratio of ventricular septal to left ventricular free were obtained. There was a significant linear correlation between the wall pressure difference ratio across the wall and the left ventricular free wall ratio. The right ventricular systolic pressure in the two groups calculated by the curvature ratio between the ventricular septum and the left ventricular free wall and measured by the right ventricular catheterization was in good agreement [mean difference was 7.8 鹵12.1 mm Hg]. Conclusion the right ventricular systolic pressure was measured by CAD software. The curvature ratio of ventricular septum to left ventricular free wall in cardiac magnetic resonance imaging is simple, rapid and reproducible. The measured curvature ratio of ventricular septum to left ventricular free wall can accurately predict the right ventricular systolic pressure in patients with idiopathic pulmonary hypertension.
