【摘要】：目的:采用血氧水平依賴(lài)功能磁共振成像(BOLD-fMRI)觀(guān)察脊髓損傷(SCI)截癱患者序列對(duì)指運(yùn)動(dòng)時(shí)的腦激活模式,探討SCI后腦運(yùn)動(dòng)功能代償重組機(jī)制。方法:12例手部感覺(jué)運(yùn)動(dòng)功能正常的SCI截癱患者(SCI組)和12例健康志愿者(對(duì)照組)行序列對(duì)指運(yùn)動(dòng)fMRI掃描,采用組塊設(shè)計(jì),30s靜息和30s右手序列對(duì)指運(yùn)動(dòng)交替進(jìn)行,重復(fù)4次,總掃描時(shí)間240s。使用SPM8軟件分析右手序列對(duì)指運(yùn)動(dòng)時(shí)個(gè)體水平、組內(nèi)及組間水平的腦激活模式,分析SCI組對(duì)側(cè)大腦激活最強(qiáng)點(diǎn)T值、對(duì)側(cè)初級(jí)運(yùn)動(dòng)皮質(zhì)(M1)激活體積與損傷平面、損傷后時(shí)間及運(yùn)動(dòng)評(píng)分之間的相關(guān)性。結(jié)果:兩組中對(duì)側(cè)激活最強(qiáng)點(diǎn)T值及M1激活體積的差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05),SCI組對(duì)側(cè)M1激活體積與損傷平面呈負(fù)相關(guān)(r=-0.807,P=0.009)。與對(duì)照組比較,SCI組對(duì)側(cè)激活最強(qiáng)點(diǎn)向后移位,雙側(cè)M1、輔助運(yùn)動(dòng)區(qū)(SMA)、島葉及頂下小葉激活增強(qiáng)。結(jié)論:脊髓損傷截癱患者遠(yuǎn)隔部位運(yùn)動(dòng)相關(guān)腦區(qū)發(fā)生代償重組,且與脊髓損傷平面相關(guān)。
[Abstract]:Objective: to observe the pattern of brain activation in patients with spinal cord injury (sci) paraplegia by using blood oxygen level dependent functional magnetic resonance imaging (BOLD-f MRI). To explore the compensatory mechanism of brain motor function after SCI. Methods 12 cases of SCI paraplegia with normal hand sensorimotor function and 12 healthy volunteers (control group) were scanned by fMRI. Two pairs of finger movements were performed alternately in 30 s rest and 30 s right hand sequences, repeated 4 times, and the total scanning time was 240 s. SPM8 software was used to analyze the brain activation patterns at individual level, intra-group and inter-group level in the right hand sequence. The activation volume and injury plane of contralateral cerebral activation point T and contralateral primary motor cortex M1 were analyzed in SCI group. Results: there was no significant difference between T value and M1 activation volume at the strongest point of contralateral activation in the two groups. There was a negative correlation between the activation volume of M1 and the injury plane in the contralateral M1 activation volume in the sci group, and the correlation between the activation volume of the contralateral M1 and the injury plane was negative correlation with that of the control group. The contralateral activation point of sci group was shifted backward. Bilateral M1, SMAA, insular lobes and inferior parietal lobules were enhanced. Conclusion: compensatory recombination occurs in the motor related brain areas of the distal region of spinal cord injury paraplegia, and is related to the spinal cord injury level.
