【摘要】：目的探討十二指腸腸壁血腫的CT、MRI及消化道造影的影像學(xué)表現(xiàn)特點(diǎn)及其診斷價(jià)值。方法回顧性分析確診為十二指腸腸壁血腫的3例患兒的CT、MRI及消化道造影的圖像。結(jié)果 1例累及十二指腸降段下部及水平段,1例累及十二指腸水平段,1例累及十二指腸升段及空腸起始段;3例均造成不同程度的不全性腸梗阻,1例存在活動(dòng)性出血。2例CT表現(xiàn)為不同的混雜等、稍高密度,1例CT表現(xiàn)為低密度。1例MRI表現(xiàn)為長(zhǎng)T_1、短T_2信號(hào),增強(qiáng)掃描囊內(nèi)無(wú)強(qiáng)化;1例MRI表現(xiàn)為等T_1、混雜T_2信號(hào),增強(qiáng)掃描囊內(nèi)可見(jiàn)小斑片狀對(duì)比劑信號(hào)。結(jié)論 CT和MRI檢查在十二指腸腸壁間血腫的診斷中具有重要價(jià)值,可以作為首選的影像學(xué)檢查方法,同時(shí)MRI檢查因其不具損傷的特性,可以作為保守治療中療效評(píng)估的首選影像學(xué)檢查方法。
[Abstract]:Objective to investigate the imaging features and diagnostic value of CT MRI and gastrointestinal angiography in duodenal wall hematoma. Methods CT findings of 3 children with duodenal wall hematoma were retrospectively analyzed. Results one case involved the lower and horizontal segment of the duodenum, one case involved the horizontal segment of the duodenum, one case involved the ascending segment of the duodenum and the initial segment of the jejunum. All the 3 cases caused different degrees of incomplete intestinal obstruction and 1 case had active hemorrhage. 2 cases had different mixed CT appearances, while 1 case with slightly high density showed low density and 1 case with MRI showed long T1. Short T _ 2 signal enhanced no enhancement in the scanning sac; In one case, MRI showed iso-T _ 1 and mixed signal of T _ s _ 2. Conclusion CT and MRI have important value in the diagnosis of duodenal intermural hematoma and can be used as the first imaging method. At the same time, MRI can be used as the first choice in the evaluation of curative effect in conservative treatment because of its noninvasive characteristics.
