目的:探討兒童胰腺創(chuàng)傷的臨床特點(diǎn)、診斷及治療方法。方法:回顧分析本中心2008年1月至2017年12月收治胰腺創(chuàng)傷患兒共49例。根據(jù)美國(guó)創(chuàng)傷外科學(xué)會(huì)胰腺損傷分級(jí)法(AAST)對(duì)胰腺創(chuàng)傷進(jìn)行分級(jí),對(duì)比胰腺創(chuàng)傷不同分級(jí)及不同治療方式患者的一般情況、輔助檢查結(jié)果、住院時(shí)間、并發(fā)癥發(fā)生情況。結(jié)果:本組49例胰腺創(chuàng)傷患兒中,Ⅰ級(jí)創(chuàng)傷11例,Ⅱ級(jí)創(chuàng)傷32例,Ⅲ級(jí)創(chuàng)傷4例,Ⅳ級(jí)創(chuàng)傷2例。單純胰腺創(chuàng)傷19例,合并其他臟器損傷30例。42例胰腺創(chuàng)傷早期采用保守治療,其中23例因合并胰腺假性囊腫出現(xiàn)梗阻、感染等癥狀行胰腺假性囊腫外引流術(shù)。5例根據(jù)胰腺創(chuàng)傷部位及創(chuàng)傷程度不同分別采取胰周引流、胰腺修補(bǔ)、胰腺空腸吻合等不同手術(shù)方式。2例分別因胃幽門斷裂和十二指腸斷裂行胃十二指腸吻合和十二指腸空腸吻合術(shù),所有患者均臨床治愈。結(jié)論:兒童胰腺損傷癥狀體征不典型,診斷需要高度的警惕和詳細(xì)的影像學(xué)檢查。影響預(yù)后的最重要因素是胰管損傷情況,輕度胰腺損傷可采取保守治療,重度胰腺損傷推薦盡早手術(shù)治療,可減少并發(fā)癥發(fā)生。
