雷公藤多甙聯(lián)合福辛普利治療小兒HSPN的療效觀察

【摘要】:本研究的為探討雷公藤多甙聯(lián)合福辛普利治療小兒過敏性紫癜性腎炎(Henoch-Schonlein purpura nephritis,HSPN)的臨床療效及相關(guān)機(jī)制。選取2013年1月~2015年1月收入我院的94例過敏性紫癜性腎炎患兒,根據(jù)隨機(jī)數(shù)字表法分為對(duì)照組和觀察組,各47例。兩組患者均給予常規(guī)內(nèi)科治療,對(duì)照組患兒在此基礎(chǔ)上給予福辛普利鈉,觀察組患兒在對(duì)照組的基礎(chǔ)上給予雷公藤多甙片。比較兩組患兒的臨床資料。治療后,比較兩組患兒免疫球蛋白水平、PT、APPT、PLT、FIB水平的變化情況。觀察組的總有效率顯著高于對(duì)照組,且復(fù)發(fā)率顯著低于對(duì)照組(p0.05)。觀察組的收縮壓、舒張壓、24 h尿蛋白量及腹痛、關(guān)節(jié)疼痛和水腫消失時(shí)間等臨床癥狀的改善均顯著優(yōu)于對(duì)照組(p0.05)。治療后,兩組患者Ig A水平均顯著降低(p0.05),且觀察組較對(duì)照組降低更明顯(p0.05);對(duì)照組PT較治療前無明顯改變(p0.05),而觀察組PT則顯著升高(p0.05),組間比較差異顯著(p0.05);兩組患者PLT較治療前均顯著降低(p0.05),且觀察組較對(duì)照組降低更明顯,組間比較差異顯著(p0.05)。雷公藤多甙聯(lián)合福辛普利對(duì)小兒HSPN臨床療效較好,可顯著改善患者癥狀,其機(jī)制可能與降低Ig A水平、抑制細(xì)胞免疫、升高PT、降低PLT計(jì)數(shù),以及改善凝血功能相關(guān)。
[Abstract]:The purpose of this study was to investigate the clinical efficacy and mechanism of tripterygium wilfordii polyglycoside combined with fosinopril in the treatment of Henoch-Schonlein purpura nephritis (Henoch-Schonlein purpura nephritis,HSPN) in children. From January 2013 to January 2015, 94 children with Henoch-Schonlein purpura nephritis (Henoch-Schonlein purpura nephritis) were divided into control group (n = 47) and observation group (n = 47). The children in the control group were given fosinopril sodium and the children in the observation group were given tripterygium wilfordii polyglycoside tablets on the basis of the control group. The clinical data of the two groups were compared. After treatment, the levels of immunoglobulin and PT,APPT,PLT,FIB were compared between the two groups. The total effective rate of the observation group was significantly higher than that of the control group, and the recurrence rate was significantly lower than the control group (p0.05). The improvement of systolic blood pressure, diastolic pressure, 24-hour urinary protein, abdominal pain, joint pain and disappearance time of edema in the observation group were significantly better than those in the control group (p0.05). After treatment, the level of Ig A in the two groups decreased significantly (p0.05), and the level in the observation group was more significantly lower than that in the control group (p0.05). The PT in the control group had no significant change compared with that before treatment (p0.05), but the PT in the observation group increased significantly (p0.05), and the difference between the two groups was significant (p0.05). The PLT of the two groups was significantly lower than that of the control group (p0.05), and that of the observation group was more obvious than that of the control group, and the difference between the two groups was significant (p0.05). Tripterygium wilfordii polyglycoside combined with fosinopril can significantly improve the symptoms of children with HSPN. The mechanism may be related to the decrease of Ig A level, inhibition of cellular immunity, increase of PT, and decrease of PLT count, as well as improvement of coagulation function.
