血漿半乳糖凝集素-3在川崎病中的水平及其與冠脈損傷的相關(guān)性研究

【摘要】:目的:觀察川崎病(kawasaki disease,KD)患兒不同時(shí)期血漿半乳糖凝集素-3(Galectin-3,Gal-3)的水平及其與冠脈病變(coronary arterial lesions,CAL)的關(guān)系。方法:測(cè)定50例KD患兒急性期(發(fā)病11天,人丙種球蛋白使用前)、亞急性期(病程的11-21天,IVIG使用后,體溫正常)和20例年齡相仿門(mén)診體檢的健康兒童的血漿Gal-3的水平,同時(shí)記錄患兒的一般臨床資料如PLT、ESR、CRP。并根據(jù)是否發(fā)生CAL分組分析患兒血漿Gal-3的水平變化,并分析其與血小板計(jì)數(shù)(PLT)、血沉(ESR)、C反應(yīng)蛋白(CRP)的相關(guān)性。結(jié)果:KD患兒急性期、亞急性期血漿Gal-3濃度分別為(14.64±1.99)ng/ml、(11.89±1.25)ng/ml,健康體檢兒童血漿Gal-3濃度為(10.15±0.79)ng/ml。KD患兒急性期、亞急性期血漿Gal-3濃度均明顯高于健康體檢兒童(P0.05);急性期明顯高于亞急性期,(P0.05);急性期冠脈病變組(CAL組,n=17)和非冠脈病變組(NCAL組,n=33)血漿Gal-3的濃度分別為(16.40±1.21)ng/ml、(13.77±1.48)ng/ml,CAL組高于NCAL組(P0.05);同時(shí)KD患兒急性期血漿Gal-3水平與PLT、CRP水平呈正相關(guān)關(guān)系(P0.05)。結(jié)論:KD患兒血漿Gal-3水平急性期顯著升高,并隨病程下降,急性期CAL組血漿Gal-3水平高于NCAL組,血漿Gal-3急性期顯著升高對(duì)于患兒冠狀動(dòng)脈損害有一定的預(yù)測(cè)意義或可輔助川崎病的診斷。
[Abstract]:Aim: to investigate the relationship between plasma galactose agglutinin 3 (Galectin-3,Gal-3) and coronary artery disease (coronary arterial lesions,CAL) in children with Kawasaki disease (kawasaki disease,KD). Methods: the acute phase (11 days of onset, before the use of human gamma globulin) and the subacute phase (11-21 days of course of disease, after the use of IVIG) were measured in 50 cases of KD. (normal body temperature) and 20 healthy children of similar age for outpatient check-up, plasma Gal-3 levels were recorded, and general clinical data such as PLT,ESR,CRP. were recorded. The level of plasma Gal-3 was analyzed according to whether CAL occurred or not, and the correlation between the level of plasma Gal-3 and (PLT), ESR (ESR), C reactive protein (CRP) was analyzed. Results: the plasma Gal-3 concentrations in children with KD were (14.64 鹵1.99) ng/ml, (11.89 鹵1.25) ng/ml, in acute phase and (11.89 鹵1.25) ng/ml, in subacute phase, respectively. The plasma Gal-3 concentration of healthy children was (10.15 鹵0.79) ng/ml.KD in acute stage, and the plasma Gal-3 concentration in subacute phase was significantly higher than that in healthy children (P0.05). The acute stage was significantly higher than the subacute stage (P0.05). The plasma Gal-3 levels in acute coronary lesion group (CAL group, nong17) and non-coronary lesion group (NCAL group, nong33 group) were (16.40 鹵1.21) ng/ml, (13.77 鹵1.48) ng/ml,CAL group and (16.40 鹵1.21) ng/ml, (13.77 鹵1.48) ng/ml,CAL group, respectively (P0.05). At the same time, there was a positive correlation between plasma Gal-3 level and PLT,CRP level in children with KD (P0.05). Conclusion: the plasma Gal-3 level in children with KD increased significantly in acute phase, and decreased with the course of disease. The plasma Gal-3 level in CAL group was higher than that in NCAL group in acute phase. The significant increase of plasma Gal-3 in acute phase may predict coronary artery damage in children or assist the diagnosis of Kawasaki disease.
