早產(chǎn)兒肺功能檢查的臨床研究

【摘要】:目的通過(guò)監(jiān)測(cè)早產(chǎn)兒生后早期和糾正胎齡滿6個(gè)月的潮氣呼吸肺功能數(shù)據(jù),比較生長(zhǎng)前后的肺功能發(fā)育狀態(tài),探討早產(chǎn)出生嬰兒肺功能發(fā)育的影響因素。方法選取60例于廣西壯族自治區(qū)人民醫(yī)院兒科住院的孕32-36+6周出生的早產(chǎn)兒作為研究對(duì)象。其中,中期早產(chǎn)兒30例;晚期早產(chǎn)兒30例。應(yīng)用德國(guó)耶格Masterscreen肺功能儀檢測(cè),分別于早產(chǎn)兒出生10天內(nèi)及糾正胎齡滿6個(gè)月進(jìn)行潮氣呼吸肺功能檢測(cè)。結(jié)果1.早產(chǎn)兒的RR為54.93±11.27(次)高于足月兒46.90±6.70(次),兩組比較有統(tǒng)計(jì)學(xué)意義(P0.05);早產(chǎn)兒PTEF(38.60±7.49)、TEF75(36.03±8.45)、TEF50(36.10±7.18)、TEF25(27.05±6.57)低于足月兒PTEF(60.67±13.32)、TEF75(55.23±13.25)、TEF50(58.87±13.27)、TEF25(48.60±10.60),兩組比較差異均有統(tǒng)計(jì)學(xué)意義(p0.05);早產(chǎn)兒TPTEF/TE(31.76±9.74)、VPEF/VE(31.61±7.64)低于足月兒TPTEF/TE(40.62±7.05)、VPEF/VE(38.32±5.88),但差異無(wú)統(tǒng)計(jì)學(xué)意義(p0.05)。2.中期早產(chǎn)兒與晚期早產(chǎn)兒兩組的TPTEF/TE、VPEF/VE、VT、VT/kg、RR、PTEF、TEF75、TEF50、TEF25比較差異均沒有統(tǒng)計(jì)學(xué)意義(p0.05)。3.中期早產(chǎn)兒糾正胎齡6個(gè)月的TPTEF/TE、VPEF/VE、VT、PTEF、TEF75、TEF50和TEF25均明顯高于出生10天內(nèi),兩組比較均有統(tǒng)計(jì)學(xué)意義(P0.01);中期早產(chǎn)兒出生10天內(nèi)的RR為52.45±12.22(次)高于糾正胎齡6個(gè)月35.55±6.74(次),兩組比較有統(tǒng)計(jì)學(xué)意義(P0.01)。4.晚期早產(chǎn)兒糾正胎齡6個(gè)月的TPTEF/TE、VPEF/VE、VT、PTEF、TEF75、TEF50和TEF25均明顯高于出生10天內(nèi),兩組比較均有統(tǒng)計(jì)學(xué)意義(P0.01);晚期早產(chǎn)兒出生10天內(nèi)的RR為51.60±10.19(次)高于糾正胎齡6個(gè)月36.47±8.10(次),兩組比較有統(tǒng)計(jì)學(xué)意義(P0.01)。結(jié)論1.早產(chǎn)兒呼吸頻率增快,呼氣流速較低,氣道阻力高。2.中期早產(chǎn)兒肺功能與晚期早產(chǎn)兒肺功能參數(shù)基本相當(dāng)。3.早產(chǎn)兒潮氣肺功能隨年齡生長(zhǎng)變化。4.潮氣肺功能檢測(cè)對(duì)早產(chǎn)兒肺發(fā)育的評(píng)估有臨床價(jià)值。
[Abstract]:Objective to investigate the influencing factors of lung function development in premature infants by monitoring the lung function data of preterm infants at early postnatal stage and correcting 6 months of gestational age and comparing the development status of lung function before and after growth. Methods A total of 60 preterm infants with 32-36 6 weeks gestation in pediatrics of Guangxi Zhuang Autonomous region people's Hospital were selected as study objects. Among them, 30 cases were intermediate premature infants and 30 cases were late premature infants. The pulmonary function of preterm infants was measured by using the Jager Masterscreen pulmonary function instrument within 10 days of birth and 6 months of corrected gestational age.
