【摘要】：目的探討單切口腹腔鏡疝囊高位結(jié)扎術(shù)在兒童復(fù)發(fā)性腹股溝疝中的應(yīng)用價(jià)值。方法回顧性分析2011年1月~2016年2月我院?jiǎn)吻锌诟骨荤R治療30例兒童復(fù)發(fā)性腹股溝疝的臨床資料。年齡1~11歲,(6.1±3.3)歲。對(duì)斜疝采用腹腔鏡下帶線針、鉤針配合,內(nèi)環(huán)口腹膜外潛行置線、荷包結(jié)扎的方法高位結(jié)扎疝囊;對(duì)直疝用類似方法結(jié)扎疝囊,同時(shí)將臍動(dòng)脈襞縫合覆蓋于直疝三角,起到加固作用。結(jié)果術(shù)中證實(shí)28例為斜疝復(fù)發(fā),其中4例對(duì)側(cè)存在隱匿性斜疝;2例為直疝。手術(shù)時(shí)間12~45 min,(21.4±8.2)min。30例隨訪2~53個(gè)月,(23.9±12.5)月,均無(wú)再次復(fù)發(fā)。術(shù)后瘢痕隱蔽,無(wú)并發(fā)癥。結(jié)論單切口腹腔鏡治療兒童復(fù)發(fā)性腹股溝疝安全有效,瘢痕隱蔽,并可術(shù)中診斷處理隱匿性疝、直疝。
[Abstract]:Objective to evaluate the value of laparoscopic high ligation of hernia sac in recurrent inguinal hernia in children. Methods the clinical data of 30 children with recurrent inguinal hernia treated by single incision laparoscopy from January 2011 to February 2016 were retrospectively analyzed. The age was 11 years old and 6.1 鹵3.3 years old. The indirect hernia was ligated by laparoscope with wire needle, crochet needle, inner ring, ventral and outer membrane, and purse ligation, and the hernia sac was ligated with similar method for direct hernia, and the umbilical artery fold was sutured over the triangle of direct hernia, and the hernia sac was ligated in the same way as the suture of umbilical artery fold in the triangle of direct hernia. Play a reinforcing role. Results 28 cases of indirect hernia recurred during operation, of which 4 cases had occult indirect hernia on the contralateral side, 2 cases were direct hernia. The operative time was 12 ~ 45 minutes and 21.4 鹵8.2)min.30 cases were followed up for 2 ~ 53 months and 23.9 鹵12.5 months. No recurrence was found. The scar was concealed and there was no complication after operation. Conclusion single incision laparoscopy is safe and effective in the treatment of recurrent inguinal hernia in children.
