延邊小學(xué)生血脂體質(zhì)量指數(shù)與代謝綜合征的關(guān)系

【摘要】:目的探討延邊地區(qū)漢族和朝鮮族小學(xué)生血清游離脂肪酸(FFA)與體質(zhì)量指數(shù)(BMI)對代謝綜合征(MS)患病危險的關(guān)聯(lián)性,為早期篩選兒童青少年MS高危人群提供依據(jù)。方法分別于2012年7月15—18日和2013年7月16—21日,在吉林省延邊朝鮮族自治州選擇5所小學(xué)校,采用整群抽樣的方法,共抽取5 747名小學(xué)生(漢族3 736名,朝鮮族2 011名)作為研究對象,全部測量身高、體重、腰圍和血壓,并檢測清晨空腹血清三酰甘油、高密度脂蛋白膽固醇、血糖、FFA。結(jié)果調(diào)整年齡和性別的協(xié)方差分析結(jié)果顯示,漢族學(xué)生MS組和正常組FFA平均水平分別為(22.72±2.72)和(21.09±2.60)μmol/L,朝鮮族學(xué)生分別為(23.49±1.64)和(21.95±1.63)μmol/L;漢族學(xué)生MS組和正常組BMI平均水平分別為(25.68±3.58)和(16.98±2.89)kg/m~2,朝鮮族學(xué)生分別為(23.11±3.63)和(17.86±3.05)kg/m~2,漢族和朝鮮族學(xué)生MS組FFA和BMI平均水平均高于正常組學(xué)生(P值均0.01),且不同民族學(xué)生MS患病水平均隨著FFA和BMI水平的增加而增加(P值均0.01)。調(diào)整了年齡和性別因素后,Logistic回歸分析顯示,漢族學(xué)生FFA水平第四分位組MS患病危險是第一分位組的2.693倍,朝鮮族學(xué)生為12.132倍;漢族學(xué)生超重組和肥胖組的MS患病危險是正常組的19.646和223.666倍,朝鮮族學(xué)生分別為4.860和19.204倍。FFAP_(75)且BMI超重及肥胖組的漢族(OR=172.524,95%CI=60.288~493.705)和朝鮮族(OR=79.188,95%CI=35.728~175.513)學(xué)生MS患病危險升高,且均高于單純FFAP_(75)組和BMI超重及肥胖組。結(jié)論延邊地區(qū)學(xué)生FFA和BMI均與MS患病密切相關(guān),FFA升高合并超重及肥胖學(xué)生MS患病危險性較高。FFA水平升高可以預(yù)測MS的患病危險,并且對于朝鮮族學(xué)生的預(yù)測作用強(qiáng)于漢族學(xué)生。
[Abstract]:Objective to investigate the association between serum free fatty acid (FFAA) and body mass index (BMI) in primary school students of Han nationality and Korean nationality in Yanbian area, and to provide evidence for early screening of high risk population of MS among children and adolescents. Methods from 15 to 18 July 2012 and from 16 to 21 July 2013, 5 primary schools were selected from Yanbian Korean Autonomous Prefecture of Jilin Province. A total of 5 747 pupils (3 736 Han nationality) were selected by cluster sampling. The height, weight, waist circumference and blood pressure were measured, and fasting serum triglyceride, high density lipoprotein cholesterol and FFAs were measured. Results the covariance analysis, adjusted for age and sex, showed that The average FFA levels of MS group and normal group were 22.72 鹵2.72 渭 mol / L and 21.09 鹵2.60 渭 mol / L, those of Korean students were 23.49 鹵1.64) and 21.95 鹵1.63 渭 mol / L respectively; the average BMI levels of MS group and normal group of Han nationality students were 25.68 鹵3.58) and 16.98 鹵2.89 kg 路mm2; Korean students were 23.11 鹵3.63 渭 mol / L and 17.86 鹵3.05 kg 路m-2; Han and Korean nationality students were 23.11 鹵3.63 渭 mol / L and 17.86 鹵3.05 渭 mol / L, respectively. The average levels of FFA and BMI in MS group were higher than those in normal group (P < 0.01), and the prevalence level of MS in students of different nationalities increased with the increase of FFA and BMI levels. After adjusting for age and sex, logistic regression analysis showed that the risk of MS in the fourth quartile group of Han nationality students was 2.693 times that of the first one group, and that of Korean students was 12.132 times. The risk of MS in Han nationality students was 19.646 and 223.666 times higher than that in the normal group, and the risk of MS in Korean students was 4.860 and 19.204 times. All of them were higher than those of FFAPD 75) group and BMI overweight and obesity group. Conclusion both FFA and BMI are closely related to the prevalence of MS in Yanbian area. The higher risk of MS in students with overweight and obesity can predict the risk of MS, and the predictive effect on Korean students is stronger than that of Han students.
