血乳酸及陰離子間隙在窒息新生兒中的應(yīng)用價值

【摘要】:目的探討窒息新生兒血乳酸、陰離子間隙變化與其窒息程度的關(guān)系,指導(dǎo)臨床治療及判斷預(yù)后。方法選擇2014年1~12月?谑腥嗣襻t(yī)院新生兒科窒息新生兒201例,納入觀察組;同期選擇無窒息、病情穩(wěn)定的新生兒205例,納入對照組。分別在入院后1h檢測血?dú)庵笜?biāo)及生化指標(biāo),按照窒息程度分組,比較各組間血?dú)庵笜?biāo)、血乳酸、AG值存在的差異。結(jié)果新生兒的性別、胎齡、體重等一般資料比較,差異無統(tǒng)計學(xué)意義(P0.05),該條件下,觀察組窒息患兒血乳酸水平、AG值均高于對照組,差異均有統(tǒng)計學(xué)意義(P0.05),重度窒息患兒1 min Apgar、血乳酸水平和AG值高于輕度窒息患兒,差異有統(tǒng)計學(xué)意義(P0.05),但兩組間患兒的p H值變化并不大(P0.05);治愈組和死亡組比較,兩組患兒的胎齡、出生體重、p H值等指標(biāo),差異無統(tǒng)計學(xué)意義(P0.05),但死亡組患兒的血乳酸、AG值高于治愈組,且死亡組患者的1 min Apgar評分明顯低于治愈組,差異均有統(tǒng)計學(xué)意義(P0.05)。結(jié)論血?dú)馊樗崴胶虯G值在窒息患兒中表達(dá)高水平,有重要的診斷價值,并且在患兒的預(yù)后評估上也有一定的參考價值。
[Abstract]:Objective to investigate the relationship between the changes of blood lactic acid and anion gap and the degree of asphyxia in asphyxia neonates. Methods 201 neonates with neonatal asphyxia in Haikou people's Hospital from January to December 2014 were included in the observation group and 205 neonates without asphyxia and stable condition were selected as control group. Blood gas index and biochemical index were detected at 1 hour after admission. According to the degree of asphyxia, the differences of blood gas index and blood lactate AG were compared. Results there was no significant difference in sex, gestational age, body weight and other general data of newborns. Under this condition, the serum lactate level in the observation group was higher than that in the control group. The difference was statistically significant (P 0.05). The level of blood lactate and AG in severe asphyxia group was higher than that in mild asphyxia group (P 0.05). However, the pH value of the two groups was not changed significantly (P 0.05), and the difference between the cured group and the death group was not significant (P 0.05). There was no significant difference in gestational age, birth weight and pH between the two groups, but the serum lactate AG value in the death group was higher than that in the cured group, and the 1 min Apgar score in the dead group was significantly lower than that in the cured group (P 0.05). Conclusion the high level of blood gas lactate and AG in asphyxia children has important diagnostic value and also has certain reference value in the prognosis evaluation of children with asphyxia.
