【摘要】：目的比較右外側(cè)小切口與胸骨正中切口治療小兒先天性心臟病的臨床效果。方法回顧性分析2013年11月一2014年12月廣西醫(yī)科大學(xué)第一附屬醫(yī)院心臟外科19例右外側(cè)小切口心內(nèi)畸形矯治術(shù)(微創(chuàng)組)和62例傳統(tǒng)正中切口心內(nèi)畸形矯治術(shù)(傳統(tǒng)組)的病例資料。結(jié)果兩組病例在手術(shù)時(shí)間、體外循環(huán)轉(zhuǎn)流時(shí)間、主動(dòng)脈阻斷時(shí)間、機(jī)械通氣時(shí)間、ICU監(jiān)護(hù)時(shí)間、術(shù)后引流量及術(shù)后住院時(shí)間比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(P值0.05)；止痛劑使用次數(shù)及切口長(zhǎng)度比較,差異有統(tǒng)計(jì)學(xué)意義(P值0.05)。術(shù)后病例均無(wú)死亡,右外側(cè)小切口組復(fù)跳時(shí)安裝臨時(shí)起搏器1例,傳統(tǒng)正中切口組術(shù)后安裝臨時(shí)起搏器2例,術(shù)后良好復(fù)律。結(jié)論右外側(cè)小切口可以安全有效的替代某些正中開(kāi)胸矯治小兒簡(jiǎn)單的先天性心臟畸形,手術(shù)切口相對(duì)隱蔽且創(chuàng)傷小,不破壞胸骨完整連續(xù)性,術(shù)后疼痛減輕,值得推廣應(yīng)用。
[Abstract]:Objective to compare the clinical effect of right lateral small incision and sternum median incision in the treatment of children with congenital heart disease. Methods from November 2013 to December 2014, 19 cases of intracardiac malformation correction with right lateral small incision and 62 cases with traditional median incision were retrospectively analyzed in the cardiac surgery department of the first affiliated Hospital of Guangxi Medical University. Case data of the United Nations. Results the operative time, cardiopulmonary bypass time, aortic occlusion time, mechanical ventilation time and ICU monitoring time, postoperative drainage volume and postoperative hospitalization time were compared between the two groups. The difference was not statistically significant (P = 0.05), but there was a significant difference in the number of times of use of painkillers and the length of incision. There was no death after operation. Temporary pacemaker was installed in the right lateral small incision group (1 case) and in the traditional median incision group (2 cases). Conclusion the right lateral small incision is a safe and effective alternative to some median thoracotomy for the correction of simple congenital heart malformation in children. The surgical incision is relatively subtle and less traumatic, does not destroy the complete continuity of the sternum, and the postoperative pain is alleviated. It is worth popularizing and applying.
