【摘要】：目的:本文采用系統(tǒng)評(píng)價(jià)方法對(duì)喜炎平注射液治療小兒支氣管炎有效性、安全性進(jìn)行分析,在meta分析基礎(chǔ)上采用成本-效果、最小成本法評(píng)價(jià)喜炎平注射液的經(jīng)濟(jì)性。為臨床合理用藥提供科學(xué)依據(jù)。方法:循證醫(yī)學(xué)系統(tǒng)評(píng)價(jià)法。以喜炎平注射液、喜炎平注射液+小兒支氣管炎、喜炎平注射液+常規(guī)治療為主題詞,檢索中國(guó)學(xué)術(shù)期刊全文數(shù)據(jù)庫(kù)(CNKI)、中文科技期刊全文數(shù)據(jù)庫(kù)(VIP)、萬(wàn)方數(shù)據(jù)庫(kù)(WangfangDate)、中國(guó)生物醫(yī)學(xué)文獻(xiàn)數(shù)據(jù)庫(kù)(CBM)、及PubMed、EMBASE、MEDLINE等數(shù)據(jù)庫(kù)。采用Meta分析方法,對(duì)喜炎平注射液治療小兒支氣管炎的有效性和安全性評(píng)價(jià)。收集315網(wǎng)站有關(guān)喜炎平注射液的價(jià)格信息,在meta分析基礎(chǔ)上采用成本-效果分析,最小成本法,評(píng)價(jià)喜炎平注射液治療小兒支氣管炎的經(jīng)濟(jì)性。結(jié)果:有效性、安全性:喜炎平注射液與炎琥寧注射液治療小兒急性支氣管炎總有效率比較:使用喜炎平注射液組總有效率與使用炎琥寧注射液組比較差異有統(tǒng)計(jì)學(xué)意義(RR= 1.34,95%CI:1.26,1.43,P0.00001),不良反應(yīng)發(fā)生率比較:使用喜炎平注射液組與使用炎琥寧注射液組比較,差異有統(tǒng)計(jì)學(xué)意義(RR=0.29,95%CI:0.15,0.55,P=0.0002);喜炎平注射液+常規(guī)療法與常規(guī)療法治療小兒毛細(xì)支氣管炎總有效率比較:使用喜炎平注射液+常規(guī)療法組與僅用常規(guī)療法組比較,總有效率差異有統(tǒng)計(jì)學(xué)意義(OR=10.34,95%CI:4.97,20.56,P0.00001);喜炎平注射液與利巴韋林注射液治療小兒毛細(xì)支氣管炎總有效率比較:使用喜炎平注射液組與使用利巴韋林注射液組比較,差異有統(tǒng)計(jì)學(xué)意義(OR=4.72,95%CI:3.11,7.18,P0.00001)。經(jīng)濟(jì)性:喜炎平注射液與炎琥寧注射液治療小兒急性支氣管炎成本-效果分析比較:使用喜炎平組C/E=123.93,使用炎琥寧組C/E=39.47,增量成本效果分析,ICER=371.98,敏感性分析,ICER= 420.75;喜炎平注射液+常規(guī)療法與常規(guī)療法治療小兒毛細(xì)支氣管炎成本-效果分析:使用喜炎平注射液+綜合療法組C/E(不含誤工費(fèi))=179.63,C/E(含誤工費(fèi))=179.63;僅使用綜合療法組C/E(不含誤工費(fèi))=1220.31,C/E(含誤工費(fèi))=2037.11。敏感性分析,使用喜炎平注射液+綜合療法組C/E(不含誤工費(fèi))=197.59,C/E(含誤工費(fèi))=197.59,僅使用綜合療法組C/E(不含誤工費(fèi))=1220.31,C/E(含誤工費(fèi))=2037.11;喜炎平注射液與利巴韋林注射液治療小兒毛細(xì)支氣管炎成本-效果分析:使用喜炎平注射液組C/E(不含誤工費(fèi))=238.57,C/E(含誤工費(fèi))=238.57,使用利巴韋林注射液組C/E(不含誤工費(fèi))=1231.7 C/E(含誤工費(fèi))=2053.68,敏感性分析,使用喜炎平注射液組C/E(不含誤工費(fèi))=262.42,C/E(含誤工費(fèi))=262.42,使用利巴韋林注射液組C/E(不含誤工費(fèi))=1231.70,C/E(含誤工費(fèi))=2053.68結(jié)論:喜炎平注射液用于治療小兒支氣管炎安全性較高,有效性和經(jīng)濟(jì)性具有一定的優(yōu)勢(shì)。尤其是在治療小兒毛細(xì)支氣管炎方面,在產(chǎn)生較高的總有效率的同時(shí)只需要較低的治療成本,所以是具有優(yōu)勢(shì)的治療方案。具有較好的臨床研究和應(yīng)用前景。但本系統(tǒng)評(píng)價(jià)所納入的研究文獻(xiàn)質(zhì)量不高,增加了本次系統(tǒng)評(píng)價(jià)結(jié)論產(chǎn)生偏倚的風(fēng)險(xiǎn)。
[Abstract]:Objective: to analyze the efficacy and safety of the yanyanping injection in the treatment of bronchitis in children by using the method of systematic evaluation. On the basis of meta analysis, the cost effectiveness and minimum cost method was used to evaluate the economy of the yanyanping injection. Ejection liquid, yanyanping injection + bronchitis, yanyanping injection + routine treatment as the subject words, retrieval of Chinese academic journal full text database (CNKI), Chinese scientific journal full text database (VIP), Wanfang database (WangfangDate), Chinese biomedical literature database (CBM), and PubMed, EMBASE, MEDLINE and other databases. Using Meta analysis party Method, evaluate the efficacy and safety of Xi Yanping injection in treating children bronchitis. Collect the price information about Xi Yanping injection on 315 websites, use the cost effect analysis and the least cost method on the basis of meta analysis, evaluate the economy of Xi Yanping injection in the treatment of children bronchitis. Results: effectiveness and safety: Xi Yanping note The total effective rate of injection and Potassium Sodium Dehydroandroandrographolide Succinate Injection in the treatment of acute bronchitis in children: the total effective rate of using the yanyanping injection group was statistically significant compared with that of the Potassium Sodium Dehydroandroandrographolide Succinate Injection group (RR= 1.34,95%CI:1.26,1.43, P0.00001), and the incidence of adverse reactions was compared: the ratio of the Yanping injection group to the use of the Potassium Sodium Dehydroandroandrographolide Succinate Injection group was compared with that of the Potassium Sodium Dehydroandroandrographolide Succinate Injection group. In the case of high total efficiency, it only requires lower cost of treatment, so it is an advantageous treatment. It has good clinical research and application prospects. However, the quality of the literature included in this system evaluation is not high, which increases the risk of bias in the conclusion of this system evaluation.
