【摘要】：目的探討在小兒腸套疊水壓灌腸復(fù)位后,超聲對(duì)假性腸套疊的診斷價(jià)值。方法 293例腸套疊患兒在超聲監(jiān)視下,行水壓灌腸治療后觀察是否出現(xiàn)假性腸套疊,總結(jié)其聲像圖特征及臨床價(jià)值。結(jié)果水壓灌腸后,15例患兒出現(xiàn)假性腸套疊,其中9例小腸性和4例回盲瓣炎性假性腸套疊,2例同時(shí)存在兩種套疊。小腸性套疊在24h內(nèi)可自行復(fù)位,回盲瓣炎性套疊經(jīng)抗炎對(duì)癥治療后恢復(fù)。假性腸套疊的包塊套入長(zhǎng)度、"同心圓"直徑均明顯小于復(fù)發(fā)性腸套疊(3.71±0.84VS 5.36±1.25,2.13±0.52VS 3.25±0.71,P均0.05)。結(jié)論小兒腸套疊灌腸復(fù)位后,可出現(xiàn)小腸性和回盲瓣炎性假性腸套疊,高頻超聲可有效識(shí)別假性腸套疊,降低短時(shí)間再次水壓灌腸或手術(shù)治療的風(fēng)險(xiǎn)。
[Abstract]:Objective to investigate the diagnostic value of ultrasound for pseudointussusception after hydroponic enema reduction in children with intussusception. Methods 293 cases of intussusception were monitored by ultrasound and observed whether pseudointussusception appeared after hydroponic enema treatment. Results there were 15 cases of pseudointussusception after hydroponic enema. Two types of intussusception were found in 9 cases of small intestine and 4 cases of ileocecal inflammatory pseudointussusception. Ileocecal inflammatory intussusception was recovered after anti-inflammatory therapy. The diameter of "concentric circle" of pseudointussusception was significantly smaller than that of recurrent intussusception (3.71 鹵0.84VS 5.36 鹵1.25 鹵2.13 鹵0.52VS 3.25 鹵0.71g P, P < 0.05). Conclusion after enema reduction of intussusception in children, the diameter of "concentric circle" is significantly smaller than that of recurrent intussusception. Small bowel and ileocecal inflammatory pseudointussusception may occur. High frequency ultrasound can effectively identify pseudointussusception and reduce the risk of short time repressure enema or surgical treatment.
