轉(zhuǎn)化糖電解質(zhì)注射液對(duì)小兒術(shù)中血糖影響的觀察研究

【摘要】:[目的]:通過(guò)觀察轉(zhuǎn)化糖電解質(zhì)注射液(IS)、乳酸鈉林格液(LR)和5%葡萄糖注射液(GI)對(duì)小兒術(shù)中血糖情況的影響,探討IS在小兒術(shù)中液體治療血糖控制中的優(yōu)勢(shì)。[方法]:選取年齡1-7歲;體重10-30Kg,性別不限;美國(guó)麻醉醫(yī)師協(xié)會(huì)(ASA)分級(jí)為Ⅰ-Ⅱ級(jí)行五官科手術(shù)的患兒60例,隨機(jī)分為3組(n=20):轉(zhuǎn)化糖電解質(zhì)注射液組(Ⅰ組)、乳酸鈉林格液組(R組)和5%葡萄糖注射液組(G組),所有患兒分別于靜脈輸液前(To)、靜脈輸液10 min (T1)、靜脈輸液30min (T2)、靜脈輸液60min (T3)4個(gè)時(shí)點(diǎn)分別記錄HR、MAP和血糖,并依次計(jì)算各時(shí)點(diǎn)血糖差值;[結(jié)果]:組間比較:3組患兒的體重、年齡、手術(shù)時(shí)間、輸液量無(wú)統(tǒng)計(jì)學(xué)差異,P0.05。3組患兒各時(shí)間點(diǎn)的MAP及HR變化差異無(wú)統(tǒng)計(jì)學(xué)意義,P0.05。與Ⅰ組相比:R組患兒在T1、T2、T3時(shí)點(diǎn)的血糖均比Ⅰ組低,P=0.004、P=0.001、P=0.004,并且T1-T0和T2-T1時(shí)點(diǎn)血糖差值有統(tǒng)計(jì)學(xué)差異,P=0.009、P=0.046。與G組相比:R組患兒在T1、T2、T3時(shí)點(diǎn)的血糖均明顯低于G組,P0.001。T1-To時(shí)點(diǎn)與T3-T2時(shí)點(diǎn)血糖差值有統(tǒng)計(jì)學(xué)差異,P0.01。與G組相比:I組液體在T1、T2、T3時(shí)點(diǎn)的血糖低于G組,P=0.034,P0.01,P0.01。各時(shí)點(diǎn)差值無(wú)統(tǒng)計(jì)學(xué)意義,P0.05。組內(nèi)比較:Ⅰ組:與To時(shí)點(diǎn)相比,患兒血糖在T1和T2時(shí)點(diǎn)均有升高,P=0.001,P=0.008;而在T3時(shí)點(diǎn)血糖有所降低,P0.05;患兒T1時(shí)點(diǎn)與T3時(shí)點(diǎn)血糖變化,T2時(shí)點(diǎn)與T3時(shí)點(diǎn)血糖變化有統(tǒng)計(jì)學(xué)差異,P=0.001和P=0.021。各時(shí)點(diǎn)患兒血糖差值無(wú)統(tǒng)計(jì)學(xué)差異,P0.05。R組:與T0時(shí)點(diǎn)相比,患兒血糖在T2和T3時(shí)點(diǎn)均有降低,P=0.02、P=0.008;余時(shí)點(diǎn)血糖變化無(wú)統(tǒng)計(jì)學(xué)差異,P0.05。T2-T1時(shí)點(diǎn)的差值同T3-T2時(shí)點(diǎn)的差值比較有統(tǒng)計(jì)學(xué)差異,P=0.022。G組:與T0時(shí)點(diǎn)相比,患兒血糖在T 1、T2、T3時(shí)點(diǎn)均明顯升高,P0.01;而與TT2時(shí)點(diǎn)相比,患兒血糖在T3時(shí)點(diǎn)有下降,P=0.029;T1-T0時(shí)點(diǎn)的血糖差值與T3-T2時(shí)點(diǎn)差值有統(tǒng)計(jì)學(xué)差異,P=0.033。[結(jié)論]:轉(zhuǎn)化糖電解質(zhì)注射液雖然會(huì)小幅升高血糖,但較葡萄糖注射液平穩(wěn)、安全,是小兒術(shù)中液體治療血糖控制的理想選擇。為避免可能出現(xiàn)的電解質(zhì)酸堿紊亂情況,建議限量控速輸注。
