【摘要】:目的:免疫組化方法檢測(cè)紫癜性腎炎(HSPN)、正常腎臟組織及微小病變型腎病(MCN)患兒腎臟組織腎小球中Sirt1、NF-κB p65表達(dá)情況,并對(duì)HSPN組Sirt1、NF-κB p65表達(dá)情況進(jìn)行相關(guān)性分析,探討Sirt1/NF-κB p65通路在紫癜性腎炎發(fā)生、發(fā)展中的作用及意義。分析紫癜性腎炎患兒腎小球中Sirt1與紫癜性腎炎病理分級(jí)、IgA沉積、血清IgA水平、尿蛋白水平、血肌酐、尿素氮等指標(biāo)的相關(guān)性,進(jìn)而探討Sirt1在兒童紫癜性腎炎對(duì)腎組織的保護(hù)作用。方法:選取2014年7月至2016年7月于鄭州大學(xué)第一附屬醫(yī)院臨床確診并于該院行腎臟穿刺活檢術(shù)的紫癜性腎炎患兒45例(n=45)作為實(shí)驗(yàn)組(HSPN組),根據(jù)蛋白尿水平不同,將HSPN組分為大量蛋白尿組和非大量蛋白尿組;根據(jù)病理級(jí)別不同,將HSPN組分為Ⅰ、Ⅱ級(jí)組,Ⅲ級(jí)及以上組。同時(shí)選取該院行腎臟腫瘤切除術(shù)的患兒10例(n=10)作為對(duì)照組;選取于該院行腎臟穿刺活檢術(shù)確診的微小病變型腎病(MCN)患兒15例(n=15)作為MCN組;收集所有患兒體重、腎臟病理報(bào)告、血肌酐、血尿素氮、24小時(shí)尿蛋白定量等臨床資料。免疫組化方法檢測(cè)各組腎臟組織Sirt1、NF-κB p65表達(dá)情況,IPP軟件測(cè)定其平均光密度值(AOD),應(yīng)用SPSS21.軟件進(jìn)行統(tǒng)計(jì)學(xué)數(shù)據(jù)分析。結(jié)果:1.Sirt1在對(duì)照組腎小球可見表達(dá),對(duì)照組與MCN組Sirt1AOD差異無統(tǒng)計(jì)學(xué)意義(P0.05),NF-κB p65在對(duì)照組腎小球表達(dá)較少,對(duì)照組與MCN組NF-κB p65AOD差異無統(tǒng)計(jì)學(xué)意義(P0.05)。2.HSPN組Sirt1AOD比對(duì)照組、MCN組低,差異有統(tǒng)計(jì)學(xué)意義(Z分別=-3.82,-4.40,P0.05)。3.HSPN組NF-κB p65AOD比對(duì)照組、MCN組高,差異有統(tǒng)計(jì)學(xué)意義(Z值分別=-4.91,-5.63,P0.05)。4.HSPN組Sirt1AOD與NF-κB p65AOD呈負(fù)相關(guān)(r=-0.885,P0.05)。5.HSPN組亞組分析:大量蛋白尿組Sirt1AOD較非大量蛋白尿組低,差異有統(tǒng)計(jì)學(xué)意義(t=-3.78,P0.05),大量蛋白尿組NF-κB p65AOD較非大量蛋白尿組高,差異有統(tǒng)計(jì)學(xué)意義(t=3.95,P0.05);Ⅰ、Ⅱ級(jí)組Sirt1AOD較Ⅲ級(jí)及以上組高,差異有統(tǒng)計(jì)學(xué)意義(t=8.88,P0.05),Ⅰ、Ⅱ級(jí)組NF-κB p65AOD較Ⅲ級(jí)及以上組低,差異有統(tǒng)計(jì)學(xué)意義(t=-7.88,P0.05)。6.Sirt1AOD與HSPN腎臟病理分級(jí)(r=-0.759,P0.05)、血清IgA水平(r=-0.442,P0.05)、尿蛋白水平(r=-0.650 P0.05)呈負(fù)相關(guān),而與血肌酐、血尿素氮、腎臟IgA沉積水平無相關(guān)性(P均0.05)。結(jié)論:1.Sirt1、NF-κB p65抑制炎癥反應(yīng)的通路,可能在紫癜性腎炎的發(fā)生發(fā)展過程中起保護(hù)作用;2.Sirt1在腎小球的表達(dá)程度可以反映紫癜性腎炎腎組織腎小球損傷的程度。
[Abstract]:Objective: to detect the expression of Sirt1- 魏 B p65 in glomeruli of children with Henoch-Schonlein purpura nephritis (HSPN), normal renal tissue and minimal change nephropathy (MCNN) by immunohistochemical method, and to analyze the correlation between the expression of Sirt1 and NF- 魏 B p65 in HSPN group.
