【摘要】：目的：提高年輕醫(yī)師對甲狀旁腺功能減退癥的認識,降低誤診率。 方法：分析1例甲狀旁腺功能減低疑似為癲癇合并肌炎患兒的臨床資料,并進行文獻復習與總結。 設計：病例報告。 資料：1例9歲女性患兒,病程中主要表現(xiàn)為間斷抽搞、全身肌肉疼痛、心悸,綜合患兒相關檢查,符合甲狀旁腺功能減低癥診斷。 干預措施：初給予積極抗感染、營養(yǎng)肌細胞、能量支持治療7天,患兒肌酶下降不明顯,加用潑尼松繼續(xù)治療后肌酶仍高。繼續(xù)給予改善肌細胞代謝治療,其心肌酶水平仍未能降至正常。后因反復抽搐第三次入院,給予補鈣、活性維生素D3等治療,病情好轉。 結果：患兒再無抽搐,心肌酶逐漸降下降,好轉出院。 結論：甲狀旁腺功能減低癥是一種發(fā)病率很低的內分泌疾病,易被誤診為癲癇,但若能及時發(fā)現(xiàn)血清離子的異常,及早行尿離子、PTH測定,并于低血鈣糾正后復查腦電圖,�？杀苊庋诱`診治。
[Abstract]:Objective : To improve the understanding of the hypothyroidism and reduce the misdiagnosis rate . Methods : The clinical data of 1 case with hypothyroid function was analyzed and the literatures were reviewed and summarized . Design : Case Report . Data : A 9 - year - old female patient , mainly characterized by intermittent withdrawal , systemic muscle pain , palpitation , and comprehensive child - related examination , was in accord with the diagnosis of hypoparathymic function . Intervention measures : initial administration of positive anti - infection , nutrition muscle cells , energy support for 7 days , the children ' s muscle enzyme was not obvious , and the muscle enzyme remained high after the treatment with prednisone . The myocardial enzyme level was still not reduced to normal . After the third hospitalization was repeated , calcium supplement and active vitamin D3 were given , and the condition was improved . Results : There was no convulsions in the children , the myocardial enzyme gradually decreased , and the better discharged . Conclusion : Parathyroid function is a kind of endocrine disease with low incidence rate . It is easy to be misdiagnosed as epilepsy . However , if the abnormality of serum ion can be found in time , urinary ion and PTH can be measured early , and the EEG can be checked after low blood calcium correction , so that the diagnosis and treatment can be avoided .
