【摘要】：白內(nèi)障即晶狀體囊或者晶狀體出現(xiàn)混濁,是導(dǎo)致犬只視力喪失的主要原因之一。白內(nèi)障患犬易碰撞障礙物,運(yùn)動(dòng)能力下降,甚至不能正常飲食,加重飼主的負(fù)擔(dān)。目前,白內(nèi)障最有效的治療方法是超聲乳化手術(shù),將超聲乳化探頭通過(guò)小切口伸入眼球內(nèi),乳化探頭發(fā)出超聲波將晶狀體擊碎,再將其吸出。要得到好的超聲乳化手術(shù)效果,術(shù)后光線需能夠到達(dá)眼底,且視網(wǎng)膜功能正常。白內(nèi)障成熟度、超聲乳化參數(shù)和不同的術(shù)后并發(fā)癥可導(dǎo)致光線進(jìn)入眼底受阻或者破壞的視網(wǎng)膜功能,影響超聲乳化手術(shù)的效果。本研究通過(guò)分析臨床上進(jìn)行超聲乳化手術(shù)的白內(nèi)障患犬的手術(shù)效果,明確以上因素對(duì)超聲乳化手術(shù)效果的影響程度,以提出白內(nèi)障手術(shù)的注意事項(xiàng)和降低不良效果的發(fā)生率。研究通過(guò)對(duì)患犬進(jìn)行眼科學(xué)檢查,包括淚液量測(cè)量、瞳孔對(duì)光反射、眼瞼反射、威嚇試驗(yàn)、炫光試驗(yàn)、熒光色素測(cè)試、眼壓測(cè)量、裂隙燈檢查、眼底鏡檢查、棉花球測(cè)試、迷宮測(cè)試和眼超聲檢查等,在術(shù)前對(duì)白內(nèi)障成熟度進(jìn)行分類并排除手術(shù)禁忌癥,在術(shù)中對(duì)不同的犬設(shè)置高低兩種不同超聲乳化參數(shù),在術(shù)后記錄患犬的視力恢復(fù)情況和并發(fā)癥情況,研究不同白內(nèi)障成熟度、超聲乳化參數(shù)和不同術(shù)后并發(fā)癥對(duì)犬白內(nèi)障超聲乳化手術(shù)效果的影響。結(jié)果如下:1.確診10例白內(nèi)障患犬。其中貴賓犬占40%,西施犬占10%,混種犬占10%,古代牧羊占10%,雪納瑞犬占10%,比熊犬占10%,博美犬占10%。60%為母犬,40%為公犬。小型犬占80%,中型犬占20%,大型犬占0%。幼犬占0%,成年犬占70%,老年犬占30%;2.11只眼睛其中未成熟期4只,成熟期4只,過(guò)熟期3只。術(shù)后未成熟期視力喪失率為0,成熟期視力喪失比例為25%,過(guò)熟期視力喪失比例達(dá)到33.3%;3.使用較高的超聲乳化參數(shù)的白內(nèi)障患犬術(shù)后有33.3%出現(xiàn)纖維蛋白滲出,而使用較低的超聲乳化參數(shù)的患犬則為0。4.白內(nèi)障并發(fā)癥包括纖維蛋白滲出、虹膜粘連、后囊混濁、視網(wǎng)膜脫離、青光眼,其中視網(wǎng)膜脫離和青光眼導(dǎo)致視力喪失。綜合以上試驗(yàn)結(jié)果表明:白內(nèi)障多發(fā)于貴賓犬,小型犬發(fā)病率多于中大型犬,成年母犬的發(fā)病率較高。白內(nèi)障成熟程度越高,術(shù)后并發(fā)癥的發(fā)生率越高,手術(shù)后視力喪失的幾率越大,手術(shù)效果越差。使用較高的超聲乳化參數(shù)眼內(nèi)炎癥較重。不同的并發(fā)癥會(huì)造成手術(shù)效果的降低甚至視力喪失,術(shù)后青光眼和視網(wǎng)膜脫離可導(dǎo)致動(dòng)物失明,要特別注意預(yù)防。
[Abstract]:Cataract, which is the lens capsule or lens opacity, is one of the main causes of visual loss in dogs. Cataract dogs are prone to collision with obstacles, reduced motor ability, or even normal diet, increasing the burden on the owner. At present, the most effective treatment for cataract is phacoemulsification. The phacoemulsification probe is inserted into the eyeball through a small incision, and the phacoemulsification probe sends out ultrasonic wave to break the lens and suck it out. In order to obtain good phacoemulsification effect, the light should reach the fundus after operation and the retinal function should be normal. Cataract maturity, phacoemulsification parameters and different postoperative complications can result in light entering the fundus of the retina which is blocked or destroyed, which affects the effect of phacoemulsification. In this study, the effect of phacoemulsification on phacoemulsification was analyzed, and the influence of the above factors on phacoemulsification was determined, in order to put forward the points for attention and reduce the incidence of adverse effects. In this study, eye examination was carried out on dogs, including tear volume measurement, pupil light reflex, eyelid reflex, intimidation test, glare test, fluorescent pigment test, intraocular pressure measurement, slit lamp test, fundus examination, cotton ball test. Labyrinth test and ocular ultrasound examination were used to classify cataract maturity and eliminate contraindications before operation. Two different phacoemulsification parameters were set for different dogs during operation. The effects of different cataract maturity, phacoemulsification parameters and postoperative complications on phacoemulsification were studied. The results are as follows: 1. Ten canines with cataract were diagnosed. Among them, 40 poodles, 10 Xi Shi dogs, 10 mixed breed dogs, 10 ancient Shepherd dogs, 10 Shennari dogs, 10 bear dogs, 100.60% Beamey dogs and 40% male dogs. Small dogs account for 80 meters, medium dogs for 20 and large dogs for 0. Puppies accounted for 0, adult dogs 70 and aged dogs 302.11 eyes, of which 4 were immature, 4 were mature, and 3 were overmature. The loss rate of visual acuity was 0 in immature period, 25 in mature stage and 33.3in overmature period. Fibrin exudation was found in 33.3% of cataract dogs with higher phacoemulsification parameters, while 0.4 in dogs with lower phacoemulsification parameters. Cataract complications include fibrin exudation, iris adhesion, posterior capsule opacity, retinal detachment, and glaucoma, where retinal detachment and glaucoma lead to loss of vision. The results showed that cataract was more common in poodles, the incidence of cataract in small dogs was higher than that in medium and large dogs, and the incidence of adult female dogs was higher. The higher the maturation of cataract, the higher the incidence of postoperative complications, the greater the chance of loss of vision after surgery, and the worse the effect of surgery. Using higher phacoemulsification parameters, intraocular inflammation was more severe. Postoperative glaucoma and retinal detachment can lead to blindness in animals, and special care should be taken to prevent them.
