川金果欖油膏治療大鼠燙傷的藥效學(xué)觀察

【摘要】:燒傷是指由各種物理或化學(xué)因素,包括熱液、蒸汽、火焰、電能、化學(xué)物質(zhì)、放射線等所造成組織損傷的病癥。針對(duì)頻發(fā)的燒傷,中藥治療的研究也在不斷深入。本課題主要觀察川金果欖油膏對(duì)大鼠燙傷創(chuàng)面的保護(hù)作用,并進(jìn)一步探討其作用機(jī)理,為促進(jìn)川金果欖油膏的臨床應(yīng)用及新劑型開(kāi)發(fā)提供堅(jiān)實(shí)的理論基礎(chǔ)。主要實(shí)驗(yàn)內(nèi)容及結(jié)果如下:1大鼠淺Ⅱ度燙傷模型的建立大鼠背部脫毛24h后,采用沸水加熱的100g砝碼分別作用于大鼠皮膚5s、10s、15s和20s,于燙傷24h后切取燙傷創(chuàng)面全層皮膚,制備病理切片,觀察皮膚表皮層及真皮層的病理變化。得出結(jié)論:燙傷5s能建立穩(wěn)定的大鼠淺II度燙傷模型,燙傷10s、15s、20s能建立不同程度的大鼠深I(lǐng)I度燙傷模型。2川金果欖油膏對(duì)大鼠燙傷創(chuàng)面的保護(hù)作用SD大鼠20只,雌雄各半,隨機(jī)分成5組,每組4只。傷后30min分別于各組燙傷部位給藥,以均勻覆蓋創(chuàng)面為宜,滅菌紗布包扎,每天給藥1次,連續(xù)給藥至創(chuàng)面完全愈合,模型組給予油膏基質(zhì)處理。以上皮完全覆蓋作為傷口完全愈合的依據(jù)。與模型組相比,高劑量組(P0.01)和中劑量組(P0.05)均能明顯縮短創(chuàng)面的愈合時(shí)間。在傷后第7d,與模型組相比,高劑量組和中劑量組愈合率顯著升高(P0.05),傷后14d亦延續(xù)這種趨勢(shì)。從整個(gè)愈合過(guò)程中各組燙傷大鼠的病理組織學(xué)切片來(lái)看,川金果欖油膏能在早期減輕炎癥細(xì)胞的浸潤(rùn),加快創(chuàng)面纖維細(xì)胞和毛細(xì)血管生長(zhǎng),后期加快表皮覆蓋,促進(jìn)創(chuàng)面愈合。3川金果欖油膏促進(jìn)大鼠燙傷創(chuàng)面愈合的作用機(jī)理SD大鼠84只,雌雄各半,其中正常組4只,其余大鼠隨機(jī)均分為5組,每組16只。燙傷后,各組分別給予藥物治療,于傷后1d、3d、7d、14d各組隨機(jī)選取4只大鼠,雌雄各半,尾靜脈采血離心得血清,切取創(chuàng)面組織標(biāo)本,保存于-80℃冰箱。通過(guò)對(duì)實(shí)驗(yàn)大鼠燙傷后1d-14d的部分生理指標(biāo)的測(cè)定發(fā)現(xiàn):傷后第3天,各組燙傷大鼠血清中IL-6含量上升趨勢(shì)明顯并達(dá)峰值,且均與正常組差異極顯著(P0.01),間接反映造模成功。從創(chuàng)面組織中SOD和MDA含量可以看出,燙傷創(chuàng)面均在早期有一個(gè)不斷加深的過(guò)程。而藥物組創(chuàng)面羥脯氨酸含量均在傷后第3天達(dá)峰,早于模型組,且達(dá)峰濃度明顯高于模型組。傷后第7天,與模型組相比,川金果欖油膏低劑量組創(chuàng)面組織含水量顯著下降(P0.05),傷后第14天效果更加明顯(P0.01);川金果欖油膏高、中、低劑量組SOD活性均與模型組差異極顯著(P0.01),各組MDA含量也均保持下降趨勢(shì),高劑量組下降幅度大,與模型組差異明顯(P0.05);各藥物組創(chuàng)面組織中EGF含量的達(dá)峰時(shí)間均早于模型組,且在傷后第7天,川金果欖油膏高、中劑量組創(chuàng)面組織中EGF的含量顯著高于模型組(P0.05);各組燙傷大鼠血清中IL-6含量隨時(shí)間延長(zhǎng)而下降,其中,與模型組相比,川金果欖油膏高劑量組(P0.01)和中劑量組(P0.05)下降趨勢(shì)更明顯;各組血清中TGF-p的含量在傷后第7天達(dá)峰,且與模型組相比,高劑量組燙傷大鼠血清中TGF-p的含量顯著升高(P0.01)。結(jié)論:1.運(yùn)用加熱5分鐘的100g砝碼致傷5s能建立穩(wěn)定的大鼠淺II度燙傷模型。2.川金果欖油膏能較好地提高燙傷不同時(shí)間點(diǎn)創(chuàng)面的愈合率,縮短燙傷創(chuàng)面的愈合時(shí)間,對(duì)燙傷創(chuàng)面顯示出較好的修復(fù)作用。3.川金果欖油膏促進(jìn)燙傷創(chuàng)面愈合的作用機(jī)制可能涉及促進(jìn)創(chuàng)面膠原生成,抑制脂質(zhì)過(guò)氧化反應(yīng),促進(jìn)細(xì)胞增殖和表皮爬行,控制部分炎癥介質(zhì)的過(guò)度合成和釋放,促進(jìn)燙傷大鼠EGF和TGF-p含量的升高。
[Abstract]:Burn is a disease of tissue damage caused by various physical or chemical factors, including hydrothermal, steam, flame, electric energy, chemical material, radiating line and so on. The study of Chinese medicine treatment for frequent burns is also in depth. This subject is mainly to observe the protective effect of the ointment on scald wounds in rats and further explore its effect. Mechanism to provide a solid theoretical basis for promoting the clinical application of Chuanjin olive oil ointment and the development of new dosage forms. The main experimental contents and results are as follows: 1 rats with shallow second degree scald model of 1 rats were established by boiling water heated 100g weights to 5S, 10s, 15s and 20s respectively, and scalded after 24h were cut to scald after scalding 24h. All layers of skin, pathological sections were prepared, and the pathological changes of skin epidermis and dermis were observed. Conclusion: scald 5S can establish a stable II degree scald model in rats, scald 10s, 15s, 20s can establish different degree of rat deep II scald model.2 Sichuan golden fruit olivan ointment to protect the scald wound of rats, SD rats 20, male and female. Through the measurement of some physiological indexes of 1d-14d after scald in experimental rats, it was found that the increasing trend of IL-6 content in the serum of scalded rats was obvious and reached the third day after injury. The peak value was significantly different from that of the normal group (P0.01), which indirectly reflected the success of the mold making. From the content of SOD and MDA in the wound tissue, the burn wound had a continuous deepening process in the early stage. The content of hydroxyproline in the wound surface of the drug group were all in the third Tianda peak in the post injury, earlier than the model group, and the peak concentration was significantly higher than that in the model group. Seventh Compared with the model group, the water content of the wound tissue in the low dose group of the Sichuan golden olive oil ointment decreased significantly (P0.05), and the effect was more obvious (P0.01) fourteenth days after injury. The activity of SOD in the medium and low dose group was very significant (P0.01), and the content of MDA in each group also kept declining, and the decrease of the high dose group was large, and the model was large. The peak time of EGF content in the wound tissue of each drug group was earlier than that in the model group, and the content of EGF in the wound tissue in the middle dose group was higher than that in the model group (P0.05) in the middle dose group seventh days after the injury, and the IL-6 content in the serum of the scalded rats decreased with the time prolonging, which was compared with the model group. The decreasing trend of the high dose group (P0.01) and the medium dose group (P0.05) was more obvious, and the content of TGF-p in the serum of each group was seventh Tianda peak after injury, and compared with the model group, the content of TGF-p in the serum of scalded rats increased significantly (P0.01). It can promote cell proliferation and epidermal crawling, control excessive synthesis and release of some inflammatory mediators, and promote the increase of EGF and TGF-p contents in scalded rats.
