豬源肺炎克雷伯氏菌分離菌株耐藥性調(diào)查及防控技術(shù)研究

【摘要】:肺炎克雷伯氏菌是一種很容產(chǎn)生耐藥性的革蘭氏陰性桿菌,泛耐藥性克雷伯氏菌是"超級(jí)細(xì)菌"的一種,人醫(yī)上對(duì)肺炎克雷伯氏菌的耐藥性研究較多,而獸醫(yī)上對(duì)該細(xì)菌還沒有足夠重視,沒有人對(duì)肺炎克雷伯氏菌進(jìn)行過系統(tǒng)的調(diào)研。本研究采用傳統(tǒng)的生化試驗(yàn)方法對(duì)新鄉(xiāng)市2012年~2013年間豬源肺炎克雷伯氏菌進(jìn)行了分離鑒定,并用PCR方法進(jìn)行了驗(yàn)證。通過藥敏試驗(yàn)紙片方法(K-B法)對(duì)分離的肺炎克雷伯氏菌進(jìn)行了耐藥性分析,并總結(jié)了綜合防控豬源肺炎克雷伯氏菌病的實(shí)施措施,現(xiàn)將主要研究結(jié)果簡(jiǎn)述如下:1.豬源肺炎克雷伯氏菌分離鑒定采集臨床病料進(jìn)行增菌,然后采用鑒別培養(yǎng)基DHL瓊脂和MIAC瓊脂對(duì)克雷伯氏菌進(jìn)行初步分離,反復(fù)純化后對(duì)細(xì)菌進(jìn)行生化鑒定,最終從82份樣品中,鑒定為克雷伯氏菌屬的有26株,克雷伯氏菌分離率為31.71%,其中產(chǎn)酸克雷伯氏菌4株,占克雷伯氏菌總數(shù)的15.4%;肺炎克雷伯氏菌22株,各亞種數(shù)量分別為:肺炎克雷伯氏菌19株,占克雷伯氏菌73.1%;臭鼻克雷伯氏菌2株,占克雷伯氏菌總數(shù)的7.7%;鼻硬結(jié)克雷伯氏菌1株,占克雷伯氏菌總數(shù)的3.8%,未確定種的克雷伯氏菌0株。與卓超等人2009年、2010年報(bào)道的關(guān)于人感染肺炎克雷伯氏菌的數(shù)據(jù)比較吻合。2.豬源肺炎克雷伯氏菌耐藥性研究為了解新鄉(xiāng)市范圍內(nèi)豬源肺炎克雷伯氏菌的耐藥情況,采用紙片法(K-B法)對(duì)臨床分離的菌株進(jìn)行藥敏試驗(yàn),并對(duì)產(chǎn)ESBLs酶和產(chǎn)AmpC酶菌株進(jìn)行監(jiān)測(cè)。結(jié)果臨床分離的豬源肺炎克雷伯氏菌22株中,產(chǎn)ESBLs酶的有7株,產(chǎn)AmpC酶的有1株,不產(chǎn)生ESBLs和AmpC酶的14株。未發(fā)現(xiàn)耐碳青霉烯類菌株。分離的所有肺炎克雷伯氏菌對(duì)一代頭孢、磺胺類藥物和四環(huán)素耐藥性較強(qiáng)。其中不產(chǎn)酶菌株對(duì)除一代頭孢、磺胺類藥物和四環(huán)素類以外的抗生素均比較敏感,產(chǎn)酶菌株對(duì)亞胺培南、氨曲南、多粘菌素B頭孢吡肟以及頭孢聯(lián)合酶抑制劑的藥物還有效。與人醫(yī)相的結(jié)果比較吻合。3.豬源肺炎克雷伯氏菌綜合防控研究通過克雷伯氏菌滅活疫苗試驗(yàn)證明了免疫可以很好的控制豬肺炎克雷伯氏菌病的流行,通過比較,經(jīng)驗(yàn)治療和藥敏試驗(yàn)篩選藥物治療的結(jié)果,證明藥敏試驗(yàn)在臨床應(yīng)用的重要性。最后總結(jié)歸納出了防控豬肺炎克雷伯氏菌病的綜合措施。以上研究為防控豬肺炎克雷伯氏菌病提供了數(shù)據(jù)資料和技術(shù)指導(dǎo)。4.結(jié)論研究表明,肺炎克雷伯菌在豬體內(nèi)確實(shí)存在。產(chǎn)ESBLs酶和AmpC酶肺炎克雷伯菌已證實(shí)可以在豬體內(nèi)分離到。本研究對(duì)于預(yù)防和控制豬肺炎克雷伯菌病進(jìn)行了一些初步的調(diào)查和研究,為獸醫(yī)工作者防控該病提供一些基礎(chǔ)數(shù)據(jù)。
[Abstract]:Klebsiella pneumoniae is a gram-negative bacilli that can produce drug resistance. Pan-resistant Klebsiella pneumoniae is one of the "super bacteria." there is much research on the resistance of Klebsiella pneumoniae to human doctors. Veterinarians have not paid enough attention to the bacteria, and no systematic investigation of Klebsiella pneumoniae has been conducted. In this study, the traditional biochemical test method was used to isolate and identify Klebsiella pneumoniae from the year of 2012 to 2013 in Xinxiang City, and PCR method was used to verify the isolation and identification of Klebsiella pneumoniae. Antimicrobial resistance of isolated Klebsiella pneumoniae was analyzed by using the method of drug susceptibility test (K-B), and the measures to prevent and control Klebsiella pneumoniae from swine were summarized. The main results are summarized as follows: 1. Isolation, identification and identification of Klebsiella pneumoniae isolated from swine, clinical materials were collected to increase the bacteria, and then DHL Agar and MIAC Agar were used to isolate Klebsiella pneumoniae. After repeated purification, the bacteria were biochemically identified. Finally, 82 samples were collected. Of the 26 strains identified as Klebsiella, the isolation rate of Klebsiella was 31.71, of which 4 were Klebsiella, accounting for 15.410% of the total number of Klebsiella pneumoniae, 22 strains of Klebsiella pneumoniae, 19 strains of Klebsiella pneumoniae. Accounted for 73.1% of Klebsiella, 2 strains of Klebsiella odorifolia, 7. 7% of the total number of Klebsiella, 1 strain of Klebsiella sinensis, 3. 8% of the total number of Klebsiella, 0 strains of Klebsiella spp. And Zhuo Chao et al. 2009, 2010 reported about human infection of Klebsiella pneumoniae data is consistent with. 2. 2. Study on Drug Resistance of Klebsiella pneumoniae isolated from Swine in order to understand the drug resistance of Klebsiella pneumoniae in Xinxiang City, the susceptibility test of clinically isolated strains was carried out by using K-B method and ESBLs producing and AmpC producing strains were monitored. Results among 22 strains of Klebsiella pneumoniae isolated clinically, 7 strains produced ESBLs enzyme, 1 strain produced AmpC enzyme, and 14 strains did not produce ESBLs and AmpC enzymes. No carbapenem resistant strains were found. All isolates of Klebsiella pneumoniae were resistant to primary cephalosporins, sulfonamides and tetracycline. The non-enzyme producing strains were sensitive to antibiotics except for the first generation of cephalosporins, sulfanilamides and tetracyclines. The enzyme producing strains were also effective against imipenem, aztreonam, polymyxin B cefepime and ceftaxime. The results were in good agreement with the results of human medicine. A comprehensive study on the prevention and control of Klebsiella pneumoniae from swine the inactivated vaccine test of Klebsiella pneumoniae proved that immunization can control the epidemic of Klebsiella pneumoniae very well. The results of drug treatment were screened by comparison, experience treatment and drug sensitivity test. To prove the importance of drug sensitivity test in clinical application. Finally, the comprehensive measures of prevention and control of Klebsiella pneumoniae were summarized. These studies provide data and technical guidance for the prevention and control of Klebsiella pneumoniae. Conclusion the study shows that Klebsiella pneumoniae does exist in pigs. Klebsiella pneumoniae producing ESBLs and AmpC enzymes have been confirmed to be isolated in pigs. In this study, the prevention and control of Klebsiella pneumoniae were investigated and studied, which provided some basic data for the prevention and control of Klebsiella pneumoniae.
