【摘要】： 心臟儲(chǔ)備指的是心臟功能上調(diào)可能性的大小,即心臟潛力的大小。心臟儲(chǔ)備的下降或喪失將導(dǎo)致不良預(yù)后,不能對(duì)付應(yīng)激事件,例如運(yùn)動(dòng)或分娩,甚至稍強(qiáng)的日�；顒�(dòng)。用薈萃分析(Meta分析)的方法對(duì)國內(nèi)外資料和課題組調(diào)研獲得的數(shù)據(jù)資料進(jìn)行統(tǒng)計(jì)學(xué)分析,評(píng)價(jià)運(yùn)動(dòng)鍛煉、女性懷孕對(duì)其心臟儲(chǔ)備的影響,并分析舒張期/收縮期時(shí)限比值(D/S)指標(biāo)在孕前女性和孕婦、以及不同孕期孕婦間的臨界值。 按Cochrane協(xié)作網(wǎng)工作手冊(cè)的要求,制定相應(yīng)的納入、排除標(biāo)準(zhǔn)及檢索策略;以PubMed為英文文獻(xiàn)主要來源,以中國生物醫(yī)學(xué)文獻(xiàn)數(shù)據(jù)庫為中文文獻(xiàn)主要來源,搜索2007年以前在國內(nèi)外公開發(fā)表的有關(guān)“心臟儲(chǔ)備”的文獻(xiàn);作者和課題組另一成員對(duì)搜索到的文獻(xiàn)進(jìn)行質(zhì)量評(píng)估,根據(jù)納入標(biāo)準(zhǔn)和排除標(biāo)準(zhǔn)確定納入薈萃分析的資料;按照循證醫(yī)學(xué)的原理和思想與5個(gè)科研協(xié)作中心合作,利用作者設(shè)計(jì)的心臟儲(chǔ)備人群調(diào)研數(shù)據(jù)分析管理系統(tǒng)進(jìn)行心音信號(hào)數(shù)據(jù)的采集;以加權(quán)均數(shù)差值(WMD)作為合并效應(yīng)量評(píng)價(jià)運(yùn)動(dòng)鍛煉、女性懷孕對(duì)其心臟儲(chǔ)備的影響;用受試者工作特征曲線(ROC曲線)評(píng)價(jià)D/S在孕前女性和孕婦、以及不同孕期孕婦間的臨界值。 根據(jù)預(yù)先確定的搜索策略對(duì)PubMed和CBM等數(shù)據(jù)庫進(jìn)行檢索,共搜索到12篇基于心音檢測心臟儲(chǔ)備的文獻(xiàn),手工搜索到1篇論著,按照納入標(biāo)準(zhǔn)和排除標(biāo)準(zhǔn)對(duì)文獻(xiàn)進(jìn)行篩選,符合條件的文獻(xiàn)共4篇,均為隨機(jī)對(duì)照研究。用RevMan4.2和SPSS11.5分析表明,運(yùn)動(dòng)員心臟儲(chǔ)備較普通人群有顯著性差異,說明心肌收縮能力儲(chǔ)備指數(shù)(CCRI)指標(biāo)用于評(píng)價(jià)普通人群和運(yùn)動(dòng)員的心臟儲(chǔ)備是有效的,運(yùn)動(dòng)訓(xùn)練有利于心臟儲(chǔ)備的增加;對(duì)納入的5個(gè)科研協(xié)作中心的數(shù)據(jù)資料進(jìn)行薈萃分析顯示,孕婦心臟儲(chǔ)備較孕前女性有明顯差異,說明D/S指標(biāo)用于評(píng)價(jià)孕前女性和孕婦的心臟儲(chǔ)備是有效的,懷孕這一應(yīng)激事件動(dòng)用了心臟儲(chǔ)備,使其備用部分減小;根據(jù)D/S的ROC曲線評(píng)價(jià)孕前女性和孕婦、早中孕期和晚孕期孕婦的D/S的臨界值分別為1.445,1.275;敏感度分別為82.4%,65.7%。 對(duì)普通人群和運(yùn)動(dòng)員、孕前女性和孕婦、不同孕期孕婦心臟儲(chǔ)備做的薈萃分析結(jié)果表明,CCRI、D/S能夠有效的評(píng)價(jià)心臟儲(chǔ)備;孕前女性和孕婦、早中孕期和晚孕期孕婦的D/S的臨界值能為采取保障孕婦心臟健康的措施提供參考依據(jù)。
[Abstract]:Cardiac reserve refers to the magnitude of the likelihood of an up-regulation of heart function, that is, the magnitude of cardiac potential. A decline or loss of cardiac reserves can lead to poor prognosis and inability to cope with stress events, such as exercise or childbirth, or even stronger daily activities. Meta-analysis (Meta analysis) was used to evaluate the effects of exercise and pregnancy on cardiac reserve. The critical values of the ratio of diastolic / systolic duration (D / S) between prepregnant women and pregnant women, as well as between pregnant women with different gestations were analyzed. According to the requirements of the Cochrane cooperation network work manual, to formulate the corresponding inclusion, exclusion standards and retrieval strategy; Using PubMed as the main source of English literature and Chinese biomedical literature database as the main source of Chinese literature, the literature on "cardiac reserve" published at home and abroad before 2007 was searched. The author and another member of the research group conduct a quality assessment of the literature searched and determine the information to be included in the meta-analysis according to the inclusion criteria and exclusion criteria; According to the principle and thought of evidence-based medicine, the author used the heart reserve research data analysis and management system designed by the author to collect the heart sound signal data in cooperation with five scientific research cooperation centers. The weighted mean difference (WMD) was used to evaluate the effect of exercise and pregnancy on the cardiac reserve. The critical values of D / S between prepregnant women and pregnant women, and between pregnant women with different gestations were evaluated by using the ROC curve. According to the pre-determined search strategy, the PubMed and CBM databases were searched, and 12 articles were searched based on cardiac sound detection, 1 paper was searched by hand, and the literature was screened according to the inclusion criteria and exclusion criteria. There were 4 articles in accordance with the conditions, all of which were randomized controlled studies. RevMan4.2 and SPSS11.5 analysis showed that there was significant difference in cardiac reserve between athletes and the general population, which indicated that the index of myocardial contractility reserve index (CCRI) was effective in evaluating the cardiac reserve of the general population and the athletes. Exercise training is beneficial to the increase of cardiac reserve. A meta-analysis of the data from the five research collaborating centers showed that the cardiac reserve of pregnant women was significantly different from that of pre-pregnant women, indicating that the D / S index was effective in evaluating the cardiac reserve of prepregnant women and pregnant women. The stress event of pregnancy uses the heart reserve to reduce the spare part; According to the ROC curve of D / S, the critical value of D / S was 1.4451.275 and the sensitivity was 82.4% and 65.7%, respectively. Meta-analysis of cardiac reserve in general population and athletes, pre-pregnant women and pregnant women with different pregnancies showed that CCRI,D/S could effectively evaluate cardiac reserve. The critical value of D / S in pre-pregnant women and pregnant women in early and late pregnancy can provide a reference for taking measures to protect the heart health of pregnant women.
