正常人表皮神經(jīng)形態(tài)計(jì)量學(xué)研究

【摘要】: 目的:通過皮膚神經(jīng)活檢,對正常人的表皮內(nèi)神經(jīng)纖維(IENFs)進(jìn)行形態(tài)學(xué)觀察,并對其進(jìn)行定量研究,測定表皮神經(jīng)纖維密度(IENFD)。比較年齡、解剖部位以及種族對表皮神經(jīng)分布的影響,探討正常人小腿遠(yuǎn)端IENFD參考值范圍。為皮膚神經(jīng)活檢在周圍神經(jīng)病診斷、病程監(jiān)測以及預(yù)后判定等方面的基礎(chǔ)研究及臨床應(yīng)用奠定基礎(chǔ)。 方法:對選自外科手術(shù)的71例正常皮膚標(biāo)本(其中包括手指指腹皮膚標(biāo)本1例)進(jìn)行皮膚神經(jīng)活檢。標(biāo)本經(jīng)10%福爾馬林常規(guī)固定后切為10μm的石蠟切片,以PGP9.5為特異性軸突標(biāo)記物進(jìn)行免疫組織化學(xué)染色。觀察皮膚神經(jīng)纖維的形態(tài)學(xué)特點(diǎn),并測定IENFD,進(jìn)行統(tǒng)計(jì)學(xué)分析。 結(jié)果:1.皮膚神經(jīng)以及觸覺小體對抗PGP9.5免疫反應(yīng)性好,IENFs在表皮內(nèi)多垂直于表皮走形,走行過程中可有分支現(xiàn)象。2.小腿遠(yuǎn)端各年齡組間IENFD比較差異無顯著性,P=0.4780.05;腕部各年齡組間IENFD比較差異無顯著性,P=0.8300.05。3.各解剖部位間IENFD比較差異有顯著性,P=0.0010.05。腕部組與上臂組及大腿組比較,差異均有顯著性(P=0.0010.05, P=0.0060.05),腕部IENFD較上臂及大腿低;小腿組與上臂組及大腿組比較,差異均有顯著性(P=0.0010.05,P=0.0050.05),小腿IENFD較上臂及大腿低。小腿遠(yuǎn)端IENFD為62.87±15.25 fibers/mm~2,其正常參考值范圍為40.55 fibers/mm~2。4.我國正常人群小腿遠(yuǎn)端IENFD低于芬蘭人(p0.05)。 結(jié)論:1.皮膚活檢測定IENFD取材方便、操作簡單、費(fèi)用低廉、有量化的觀察指標(biāo),故有很大臨床應(yīng)用價(jià)值;2.正常人IENFs越過真表皮交界處后,向上垂直走形,根據(jù)分支形態(tài)可分為四種類型。3.國人小腿遠(yuǎn)端IENFD平均值為62.87±15.25 fibers/mm~2,正常值參考范圍為40.55 fibers/mm~2; 4.年齡對小腿遠(yuǎn)端和腕部的IENFD無明顯影響,不同解剖部位IENFD不同,肢體近端部位IENFD較遠(yuǎn)端高;5.種族可能對IENFD有影響,我國人群小腿遠(yuǎn)端IENFD較芬蘭人低。
[Abstract]:Objective: to observe the morphology of intraepidermal nerve fiber (IENFs) in normal human by skin nerve biopsy, and to measure the density of epidermal nerve fiber (IENFD).) by quantitative study. To compare the influence of age, anatomical location and race on the distribution of epidermal nerve, and to explore the range of IENFD reference value of distal leg in normal subjects. To lay a foundation for the basic research and clinical application of skin nerve biopsy in the diagnosis, course monitoring and prognosis determination of peripheral neuropathy. Methods: skin nerve biopsy was performed in 71 normal skin samples (including 1 finger skin specimen). The specimens were fixed with 10% formalin and then cut into 10 渭 m paraffin sections. PGP9.5 was used as the specific axon marker for immunohistochemical staining. The morphological characteristics of skin nerve fibers were observed, and the IENFDs were measured for statistical analysis. The result is 1: 1. Skin nerves and tactile corpuscles counteract the immunoreactivity of PGP9.5. Most of them are perpendicular to the epidermis in the epidermis, and the branching phenomenon can occur in the course of walking. There was no significant difference in IENFD between different age groups of distal leg, and there was no significant difference in IENFD between different age groups in wrist. There was a significant difference in IENFD between different anatomical sites (P < 0. 0010.05). There were significant differences between the wrist group and the upper arm group and the thigh group (P0. 0010.05, P0. 006 0. 05). The IENFD of the wrist was lower than that of the upper arm and thigh, and that of the leg group was significantly lower than that of the upper arm and thigh group (P 0. 0010. 05). The IENFD of the lower leg was lower than that of the upper arm and thigh. The IENFD of distal leg was 62.87 鹵15.25 fibers / mm-2, and its normal reference value was 40.55 fibers / mm-1 / mm-2. The IENFD of distal leg in normal population was lower than that in Finns (p0. 05). Conclusion 1. The measurement of IENFD by skin biopsy is convenient, easy to operate, low cost and has quantitative observation index, so it has great clinical application value. After crossing the junction of the true epidermis, the normal human IENFs can be divided into four types. 3. The average IENFD of distal leg in Chinese is 62.87 鹵15.25 fibers / mmm-2, and the normal reference range is 40.55 fibers / mmm-2; 4. Age had no significant effect on IENFD of distal leg and wrist. IENFD was different in different anatomical sites, and IENFD in proximal extremity was 5% higher than that in distal extremity. Race may have an effect on IENFD and IENFD of distal leg in Chinese is lower than that in Finn.
