【摘要】:目的 1.探討少、弱精子癥與精子形態(tài)之間的關(guān)系,以及少、弱精子癥中精子形態(tài)缺陷的類(lèi)型及發(fā)生率。為不育癥的診斷及所需采取的輔助生殖技術(shù)提供依據(jù)。 2.探討少、弱、畸形精子癥和精子核成熟性的關(guān)系以及精子頭部形態(tài)缺陷和精子核成熟性的關(guān)系。 3.探討人精子頂體酶與男性不育之間的關(guān)系。 4.探討精子單純形態(tài)學(xué)畸形患者的精液細(xì)胞成分。 方法 根據(jù)世界衛(wèi)生組織(WHO)標(biāo)準(zhǔn)標(biāo)本要求禁欲3~7天在本中心取精室通過(guò)手淫方法采集后立即在37.5℃水浴箱中靜置液化,行精液常規(guī)分析。液化后標(biāo)本分別作精子改良巴氏染色檢查、精子苯胺藍(lán)染色檢查、明膠法測(cè)定精子頂體酶活性以及流式細(xì)胞儀分析精液細(xì)胞成分。 結(jié)果 1.少精子癥患者的正常形態(tài)精子百分率的中位數(shù)約為4.00,正常密度者中位數(shù)約為8.21,少精子癥組和正常密度組的正常形態(tài)精子比率的差別有統(tǒng)計(jì)學(xué)意義。 2.弱精子癥患者正常形態(tài)精子百分率的中位數(shù)為6.67,精子活力正常組則為10.89,兩者差別有統(tǒng)計(jì)學(xué)意義。 3.不孕癥中畸形精子癥的發(fā)生率約為75%;精子頭部缺陷的發(fā)生率約為70%,各種頭部缺陷中空泡頭畸形的發(fā)生率占首位,約為20%。 4.少精子癥組精液苯胺藍(lán)染色陽(yáng)性率的中位數(shù)為12(P=0.035),弱精子癥組的中位數(shù)為8(P=0.947),畸形精子癥組的中位數(shù)為10(P=0.033)。 5.少精子癥患者精液苯胺藍(lán)染色陽(yáng)性率與錐形頭畸形呈正相關(guān)(r=0.635),,畸
