椎間盤源性下腰痛動物模型的建立及行為學觀察

【摘要】:目的:建立椎間盤源性下腰痛的動物模型,探討其發(fā)病機制,觀察椎間盤源性下腰痛動物的行為學表現(xiàn)。 方法和研究內(nèi)容:實驗用雄性SD大鼠共96只,隨機分為以下三組:對照組(假手術(shù)組)、手術(shù)一組、手術(shù)二組。全身麻醉后取仰臥位,采用經(jīng)腹腔正中入路,暴露L6-7椎間盤。于椎間盤前側(cè)正中點以直徑0.2mm的特制針頭刺入L6-7椎間盤至后側(cè)外層纖維環(huán)下方,深度控制為3mm(手術(shù)一組);蛟谂c第一針道各交叉呈30度角左右分別穿刺至后外側(cè)纖維環(huán)下方(手術(shù)二組)。各時相點分別觀察自發(fā)性行為學表現(xiàn)。取L6-7椎間盤,觀察病理學改變,采用免疫組織化學方法檢測椎間盤內(nèi)CD34、IL-1β和COX-2的表達情況。 主要結(jié)果: 1.正常椎間盤髓核內(nèi)可見纖維組織樣細胞,交織成網(wǎng)狀。術(shù)后髓核內(nèi)纖維組織樣細胞透明樣變性,與膠凍狀物交織在一起,內(nèi)層纖維軟骨細胞變性。 2.手術(shù)椎間盤內(nèi)可見由前方至后方的裂隙,后側(cè)的外層纖維環(huán)未破裂,該表現(xiàn)符合椎間盤源性下腰痛的病理表現(xiàn)。 3.術(shù)后可在內(nèi)層纖維環(huán)內(nèi)層見到散在的、染色較強的CD34陽性細胞,提示有血管內(nèi)皮細胞形成。 4.術(shù)后內(nèi)層纖維環(huán)可見多處、散在棕染IL-1β陽性細胞,呈環(huán)形分布,外層纖維環(huán)很少或無陽性細胞。 5.術(shù)后髓核內(nèi)多數(shù)、散在纖維組織樣細胞胞漿棕染呈COX-2陽性,少數(shù)內(nèi)層纖維環(huán)纖維軟骨細胞可見稀疏、淡染陽性著色。 6.術(shù)后手術(shù)二組實驗大鼠的靜止不動時間延長、運動時間減少,同假手術(shù)組相比,有統(tǒng)計學意義。
[Abstract]:Objective: to establish an animal model of discogenic low back pain, to investigate its pathogenesis and to observe the behavior of low back pain caused by intervertebral disc. Methods: 96 male Sprague-Dawley rats were randomly divided into three groups: control group (sham operation group), operation group and operation group. After general anesthesia, supine position was taken, and L-6-7 disc was exposed via the median peritoneal approach. The L6-7 disc was punctured with a special needle with diameter 0.2mm at the anterior and middle point of the disc to the posterior outer fibrous ring. The depth of the disc was controlled as 3mm (surgical group). Or at 30 degrees angle with the first needle, puncture to the posterolateral fibrous ring respectively (operation group 2). Spontaneous behavior was observed at each time point. The expressions of IL-1 尾 and COX-2 in intervertebral discs were detected by immunohistochemical method. Main results: 1. Fibrous tissue-like cells can be seen in the nucleus pulposus of normal intervertebral disc, intertwined into reticular form. After operation, fibroid-like cells in the nucleus pulposus showed hyaline denaturation, intertwined with gelatinous material, and denaturation of fibrochondrocytes in the inner layer of the nucleus pulposus. 2. 2. A fissure was seen in the intervertebral disc from the front to the rear, and the outer fibrous ring of the posterior side was not ruptured, which was consistent with the pathological manifestation of low back pain caused by intervertebral disc. Scattered and strongly stained CD34 positive cells could be seen in the inner layer of the inner fibrous ring after operation, indicating the formation of vascular endothelial cells. 4. There were many spots in the inner fibrous ring, scattered in the brown stained IL-1 尾 positive cells, distributed in a circular pattern, and there were few or no positive cells in the outer fibrous ring. After operation, most of the nucleus pulposus were stained with COX-2 positive in cytoplasm of scattered fibrous tissue like cells, a few fibrochondrocytes in the inner layer were sparse and light staining positive staining was 6. 6%. The time of immobility was prolonged and the time of exercise was decreased in the two groups after operation, which was statistically significant compared with the sham operation group.
