【摘要】： 目的:對(duì)寰枕關(guān)節(jié)進(jìn)行形態(tài)學(xué)觀測(cè),探討寰枕關(guān)節(jié)后路經(jīng)關(guān)節(jié)螺釘固定通道的入釘點(diǎn),瞄定點(diǎn),釘?shù)澜嵌燃奥葆旈L(zhǎng)度。方法:觀察枕骨髁和寰椎干燥標(biāo)本并測(cè)量相關(guān)解剖數(shù)據(jù),再依據(jù)相關(guān)參數(shù)在新鮮枕頸部尸體標(biāo)本上行手術(shù)模擬置釘,術(shù)后攝片并行尸檢以驗(yàn)證螺釘位置。結(jié)果:寰枕關(guān)節(jié)后路經(jīng)關(guān)節(jié)螺釘固定的標(biāo)準(zhǔn)內(nèi)傾角為20.3°±2.0°,標(biāo)準(zhǔn)上傾角為49.4°±2.4°,螺釘在枕骨髁內(nèi)的適宜長(zhǎng)度為13.94±0.96mm,在寰椎內(nèi)的適宜長(zhǎng)度為12.84±0.62mm。模擬置釘后見(jiàn)螺釘均通過(guò)寰枕關(guān)節(jié)起到固定作用且走行于舌下神經(jīng)管下方,瞄定點(diǎn)中有68.75%在枕骨髁前上方近似于三角形的最佳區(qū)域內(nèi)。結(jié)論:寰枕關(guān)節(jié)后路經(jīng)關(guān)節(jié)螺釘固定在解剖上可行,以寰椎后弓與側(cè)塊下關(guān)節(jié)突移行處中點(diǎn)作為入釘點(diǎn),把握標(biāo)準(zhǔn)內(nèi)傾角及上傾角,選擇適宜長(zhǎng)度的螺釘則能有效完成寰枕關(guān)節(jié)后路經(jīng)關(guān)節(jié)螺釘固定。
[Abstract]:Objective: To observe the morphological observation of atlantooccipital joint, explore the point of entry, point of sight, angle and length of screw in the fixation channel of atlantooccipital posterior approach. Methods: To observe the dry specimens of the occipital condyle and Atlas and to measure the related anatomical data, and then to simulate the operation of the fresh occipital and cervical cadavers according to the relevant parameters. Results: the standard internal obliquity of the posterior atlantooccipital joint through the articular screw was 20.3 + 2 degrees, the standard obliquity was 49.4 + 2.4 degrees, the appropriate length of the screw in the occipital condyle was 13.94 + 0.96mm, and the suitable length of the atlas was 12.84 + 0.62mm. after the atlantooccipital joint. To the fixed effect and walk below the sublingual nerve canal, 68.75% of the occipital condyles are located in the best area of the triangle. Conclusion: the posterior approach of the atlantooccipital joint is anatomically feasible, and the posterior arch of the atlas and the lower joint of the lateral mass are taken as the point of the nail, and the standard inclination angle and the obliquity are selected. Appropriate length screws can effectively achieve atlantooccipital joint posterior articular screw fixation.
