【摘要】:目的 (1)對頸椎定點(diǎn)旋轉(zhuǎn)手法作用時(shí)拇指最大推扳力進(jìn)行定量研究,為推拿手法的數(shù)字化提供依據(jù);為進(jìn)一步進(jìn)行頸椎定點(diǎn)旋轉(zhuǎn)手法的計(jì)算機(jī)數(shù)字化研究提供生物力學(xué)的加載條件。(2)在計(jì)算機(jī)上建立一個(gè)頸椎各結(jié)構(gòu)的三維數(shù)字化模型,為推拿手法的直觀的可視化研究,提供一個(gè)平臺。(3)模擬旋轉(zhuǎn)手法作用,對其解剖位置、位移的變化進(jìn)行實(shí)時(shí)監(jiān)測,,用以研究手法作用的機(jī)理,探討提高手法療效的方法,減少手法的意外和損傷。(4)探討進(jìn)行頸椎定點(diǎn)旋轉(zhuǎn)手法時(shí)正常頸椎椎體、終板和后部結(jié)構(gòu)的內(nèi)在應(yīng)力及位移分布的特點(diǎn),以分析手法的機(jī)理、安全性與合理性。 方法 (1)對46名受試者進(jìn)行頸椎坐位旋轉(zhuǎn)手法操作,應(yīng)用壓力傳感器檢測系統(tǒng),檢測并記錄出現(xiàn)咔噠聲響時(shí)術(shù)者拇指頂推受試者第四頸椎棘突的最大推扳力。(2)使用螺旋CT,以1mm的間隔,對1具男性青年新鮮尸體的頸椎標(biāo)本沿軸向進(jìn)行斷層掃描。頸椎的CT圖像輸入計(jì)算機(jī),利用Matlab6.0提取圖片中輪廓線數(shù)據(jù)矩陣,轉(zhuǎn)換為dat文檔后輸入ANSYS8.0建模。(3)分析:在Ansys 8.0系統(tǒng)中加載邊界條件,計(jì)算和分析手法作用時(shí)正常頸椎椎體、終板和后部結(jié)構(gòu)、椎間盤的位移和內(nèi)在應(yīng)力的變化過程。計(jì)算頸椎各結(jié)構(gòu)的應(yīng)力和位移的大小,顯示頸椎結(jié)構(gòu)的內(nèi)部應(yīng)力和位移的分布情況。(4)在模型上刪除寰樞椎以外的骨性實(shí)體模型,以利于直接觀察和判斷寰樞椎之間發(fā)生旋轉(zhuǎn)時(shí)解剖位置的相對變化。取樞椎棘突頂點(diǎn)(A點(diǎn))、齒突垂直軸心(B點(diǎn))以及兩者之間連線的中點(diǎn)
[Abstract]:Objective 1) to quantitatively study the maximum push and pull force of the thumb when the cervical spine rotates at a fixed point, so as to provide the basis for the digitization of massage manipulation. In order to provide biomechanical loading conditions for further research on the computer digitization of cervical spine rotation manipulation, a three-dimensional digitized model of cervical vertebrae structures is established on the computer, which can be used to visualize the manipulation of massage. To provide a platform for simulating rotation manipulation, real-time monitoring of its anatomical position and displacement, in order to study the mechanism of manipulation and to explore the methods to improve the therapeutic effect of manipulation. To explore the characteristics of stress and displacement distribution of normal cervical vertebrae, endplate and posterior structure in order to analyze the mechanism of manipulation. Safety and reasonableness. Methods 46 subjects were operated with rotation manipulation in the cervical vertebra sitting position, and the pressure sensor system was used. To detect and record the maximum pushing force of the fourth cervical spine process of the subjects with thumb pushing when the click sound occurred, the cervical vertebrae specimens of a young male cadaver were scanned by axial tomography using spiral CTs at the interval of 1mm. The CT image of cervical vertebrae was input into computer, the contour data matrix was extracted by Matlab 6.0, and then converted into dat document, and then input into ANSYS8.0 modeling. 3) Analysis: loading boundary condition in Ansys 8.0 system, calculating and analyzing normal cervical vertebrae when manipulative action was done. The process of changes in endplate and posterior structure, displacements and internal stresses of the intervertebral disc. The size of stress and displacement of each structure of the cervical vertebrae was calculated, indicating the distribution of internal stresses and displacements of the cervical vertebrae.) the model of bony entity other than atlantoaxial vertebrae was removed from the model. It is helpful to observe and judge the relative position of atlantoaxial rotation directly.
