糖皮質激素和生長激素對內毒素損傷中性粒細胞凋亡的影響

【摘要】:[目的] 目前膿毒癥和膿毒癥性休克仍然是危重病人的主要死亡原因之一,特別是在嬰幼兒由于其自身發(fā)育特點,膿毒癥和膿毒癥性休克的死亡率更是居高不下。傳統的治療方法并不能明顯改善其預后,降低死亡率。進一步探討其發(fā)病機理,尋求有效的治療方法是十分必要的。大量研究認為中性粒細胞在膿毒癥時發(fā)生凋亡延遲,導致炎癥反應過度,造成組織損傷是膿毒癥發(fā)展惡化的原因之一。以往研究認為糖皮質激素具有拮抗內毒素的作用,但最近研究發(fā)現體外條件下糖皮質激素對中性粒細胞的凋亡有延遲作用。那么,究竟糖皮質激素在膿毒癥時對內毒素損傷后的中性粒細胞有何作用,是保護還是損傷?這是本實驗的目的之一。生長激素是一種蛋白質激素,以往一直被用來治療垂體性侏儒,但最近許多文獻報道生長激素在危重癥中有積極的作用,能提高膿毒癥動物的生存率。那么生長激素對內毒素損傷后中性粒細胞的凋亡是否也存在影響,這一影響在膿毒癥中的意義如何呢?這是本實驗的另一個目的。我們希望通過本實驗為臨床進一步探討膿毒癥和膿毒癥性休克的治療提供參考。 [方法] 采用不連續(xù)密度梯度離心法分離人中性粒細胞于CO_2培養(yǎng)箱(37℃,5%CO_2)中培養(yǎng)過夜。實驗共分8組(n=3),分別為正常對照組(N),內毒素組(LPS),大劑量地塞米松組(DEX1),大劑量地塞米松治療組(LPS+DEX1),小劑量地塞米松組(DEX2),小劑量地塞米松治療組(LPS+DEX2),生長激素組(GH),生長激素治療組(LPS+GH),其中LPS的終濃度為10ug/ml,DEX1的終濃度為10~(-6)M,DEN2的終濃度為10~(-8)M,GH的終濃度為100ug/ml。每組均以3小時,6小時,12小時,24小時作為檢測時點。根據實驗設計于96
[Abstract]:[objective] sepsis and septic shock are still one of the main causes of death in critically ill patients, especially in infants due to their own developmental characteristics, sepsis and septic shock mortality is even higher. Previous studies have shown that glucocorticoids have an antagonistic effect on endotoxin, but recent studies have found that glucocorticoids have delayed the apoptosis of neutrophils in vitro. Growth hormone (GH) is a protein hormone, which has been used to treat pituitary dwarfism in the past. However, it has been reported in many recent literatures that it plays an active role in critical illness and can improve the survival rate of septic animals.
